Outpatient treatment of perianal warts by scissor excision.

Abstract: SUMMARY A method of scissor excision of perianal warts, which is suitable for use in outpatient departments, is described. The low recurrence rate (two of 24 patients) three months after treatment is encouraging.

“…These findings are in line with previous studies reporting a clearance rate of 60%-100% and a recurrence rate of 3%-77%. 18,27,36,37 In a recent retrospective study comparing the Holmium laser, cryotherapy, conventional surgery and podophyllin, Ghiasy et al found that the Holmium laser was the best treatment of genital and meatal warts, with the highest clearance rate (92.2%) through a single session and the lowest recurrence rate (14.3%), compared to other treatments. 25 However, the follow-up period of this study was only 6 months, and the patients treated by the holmium laser were 39.…”

“…Previous trials have reported a cure rate of 60%-100%, 60%-97%, 56%-93.3% and 19%-80% when genital warts were treated with holmium laser, cryotherapy, conventional surgery and podophyllin respectively. 20,24,25 Treatment with podophyllin and cryotherapy are simpler procedures than other treatments while holmium laser needs a laser machine and fibre. In terms of recurrence rate, this study shows a recurrence rate of 14.3%, 33.4%, 34.2 and 71.8% when lesions removed by holmium laser, cryotherapy, conventional surgery and podophyllin respectively.…”

“…If the anal canal or urethra is involved, staged treatments may be necessary to prevent stenosis. With a view to minimising posttreatment scarring, Thompson [55] described a scissor excision technique, subsequently adapted for use in out-patient departments, [56] in which islands of epithelium are left between treated areas. Infectious HPV DNA may be inhaled in smoke generated during electro-or laser-surgery [57,58].…”
