Outpatient treatment of COVID-19 with steroids in the phase of mild pneumonia without the need for admission as an opportunity to modify the course of the disease: A structured summary of a randomised controlled trial

Abstract: Objectives: The aim of this study is to explore the effectiveness and safety of oral corticosteroids (prednisone) in the treatment of early stage SARS-Cov-2 pneumonia in patients who do not yet meet hospital admission criteria. Trial design: Randomized clinical trial, controlled, open, parallel group, to evaluate the effectiveness of steroids in adult patients with confirmed COVID-19, with incipient pulmonary involvement, without hospital admission criteria. Patients will be stratified by the presence or not o… Show more

“…Salinas et al and Saiz-Rodríguez et al studied the effect of early use of corticosteroids in non-critical COVID-19 patients, the primary result indicated serious side effect to patients such as respiratory deterioration, worsening of the glycemic profile, delirium, pneumonia, urinary tract infection, device-associated infections, need to admission to ICU, mechanical ventilation or death by day 28. The COVID-19 pandemic has been reported with very high incidence of mucormycosis (black fungus infection) among COVID-19 patients, especially in those who have had treatment with higher doses or longer courses of steroids or who are diabetic or cancer or HIV or have weak immune systems and have received steroids [ 68 , 69 ].…”

“…In our sample, the patients treated with corticosteroid therapy had higher severity of delirium. Although this association is commonly reported in other medical conditions [ 32 , 33 ], our study is the first one to assess the association in patients with COVID-19. This finding is relevant, considering the neuroinflammation hypothesis of the “cytokine storm” [ 2 , 9 ], which contributes to neurological impairment in COVID-19 [ 2 , 8 ].…”

“…In this case, the patient’s hemoglobin and liver function were dramatically improved on the maintenance dose of prednisolone. A recent clinical trial postulated the efficacy of corticosteroids in managing COVID-19 patients; however, its impact on viral clearance and risk of other infections is a matter of controversy [ 14 16 ].…”
