Outpatient management of severe ovarian hyperstimulation syndrome: a systematic review and a review of existing guidelines

Abstract: Ovarian hyperstimulation syndrome (OHSS) is a potentially serious complication of assisted reproductive treatment. Management of women with severe OHSS has traditionally included hospitalisation for close monitoring and supportive treatment. The aim of this review is to assess the evidence for safety and efficacy of outpatient management of severe OHSS. A systematic review of studies describing outpatient management options was performed. Current guidance from advisory bodies was also reviewed. Outpatient mana… Show more

“…The treatment approach for the clinical management of OHSS is multifaceted and individualized based on disease severity and progression. Most mild and moderate cases of OHSS are self-limited and require only intensive monitoring in the outpatient department; however, severe OHSS requires hospitalization to relieve symptoms and control the progression of the disease ( 23 , 24 ). In this study, TAT, PIC, and sTM levels in the mild–moderate OHSS group were significantly higher than those in the control group ( P < 0.001, P < 0.001, and P < 0.001, respectively).…”

“…Paracentesis of ascitic fluid, rehydration and thromboprophylaxis were the main components of therapy. 89 A rare but severe complication of high grade OHSS is hydrothorax, located mainly on the right side, due to transfer of the abdominal fluid to the chest through the thoracic duct. Therefore, when dyspnea occurs, an appropriate diagnostic path must be followed with physical examination, chest radiography and arterial blood gas analysis.…”
