Outcomes of the Kaiser Permanente Tele-Home Health Research Project

Abstract: Remote video technology in the home health care setting was shown to be effective, well received by patients, capable of maintaining quality of care, and to have the potential for cost savings. Patients seemed pleased with the equipment and the ability to access a home health care provider 24 hours a day. Remote technology has the potential to effect cost savings when used to substitute some in-person visits and can also improve access to home health care staff for patients and caregivers. This technology can … Show more

“…15 Various studies report acceptance of telemedicine by patients as being high. [16][17][18] The descriptions produced report that more frequent contact with their own health affects patient adherence and self-monitoring activities. 19 One study reported that patients with diabetes who received a more welcoming service were more likely to perform preventive measures, selfcare and better blood glucose control than patients without such attention.…”

“…These technologies can take any number of forms such as web-based applications, 3 mobile phone and alert systems, 4,5 telephone and video conferencing with patients, 6,7 as well as any combination of these applications. In the last five years there has been a rapid increase in the use of monitoring technology as a management tool for long-term conditions despite a lack of robust evidence of clinical benefit to patients.…”

“…Primarily, the reduction of current service cost seems to warrant the attention of a number of studies. 7,33 It is unclear from some of the published studies on technology whether researchers were required to pay for the equipment used to undertake the research. The added costs of providing telecommunications and additional equipment such as weight scales 47 to these patients are unknown, as are the costs for the ubiquitous technology upgrades and criteria such as which patients should have the service and over what period of time is required for maximal clinical improvement.…”

“…In a quasi-experimental study, Johnston et al 25 studied the use of remote video technology with 212 chronically ill patients. They found no differences in the quality indicators of medication compliance, knowledge of disease, or self-care ability; patient satisfaction; or service use.…”

“…Three larger studies 2,6,25 showed that although initially care delivery costs were increased due to the cost of equipment, overall costs of care were decreased due to decreased hospital charges and travel costs. In most studies, small sample sizes and lack of robust economic analyses preclude generalization of financial results.…”
