Outcomes of surgery for acromioclavicular joint dislocation using different angled hook plates: a prospective study

Abstract: Hook plate treatment for AC joint dislocation can achieve the desired results, but the efficacy was significantly different depending on the different angles of the hook plate. AHP should be controlled within the range of 0-40° as much as possible when making clinical decisions.

“…Some factors related to postoperative shoulder pain had been proposed, mainly focusing on the hook plate, such as the clavicle hook hit the shoulder, the hook tail was too long and the poor hook position, etc. It was recommended to remove the clavicular hook plate as soon as possible for early functional exercise and rehabilitation (8,13). However, there were no systematic and detailed clinical studies on the factors affecting postoperative shoulder pain.…”

“…According to relevant literature reports (13)(14)(15)(16)(17) and the clinical experience of the research team, this study included a total of 13 potential risk factors Age, Gender, body-mass index (BMI), smoking status, alcohol consumption, Type of injury, Rockwood Classification, Site of injury, Operation time, Injury-to-surgery, DHA, DH and AHP (Table 1). Age was divided into 3 categories: less than or equal to 30 years old, more than 30 years old and less than 60 years old, more than or equal to 60 years old.…”

“…The truly anatomic reconstruction of the trapezoid ligament and the conoid ligament might successfully restore the stabilization of the AC joint. Rigid fixation techniques like Kirschner wires, screws and clavicular hook plates could not restore the anatomic structure of the CC and AC ligaments and were always required a secondary operation [15]. The Endobutton was first used for the reconstruction of the CC ligaments in 2007 and showed a reliable result for the treatment of AC joint dislocation [16].…”

“…It can generate stable pressure to accelerate the healing of acromioclavicular and coracoclavicular ligament. [4] Nevertheless, the single use of clavicular hook plate can easily result in the occurrence of post-operative complications, such as acromioclavicular joint re-dislocation. Therefore, to explore an optimal treatment method is a clinically concerned focus.…”
