Outcomes of sphincterotomy for small pupil phacoemulsification

Abstract: The incidence of post-operative complications following uncomplicated phacoemulsification and IOL implantation with pupillary sphincterotomy is low. The most important predisposing factors for development of a complication are ocular co-morbidities such as glaucoma, uveitis and the presence of a macular pathology.

“…However, postoperative complications included transient ocular hypertension (4%), sustained ocular hypertension (1%), persistent uveitis of more than 1 month (4%), and cystoid macular edema (5%). [ 32 ] In our series, no such significant adverse events were encountered, apart from the single unrelated case of zonular dialysis. This relative lack of complications suggests that the use of a PED such as the I-Ring may be less traumatic and less inflammatory than pupil enlargement by cutting the iris.…”

“…[9][10][11] The use of sphincterotomies for pupil enlargement has generally been abandoned by most surgeons despite some reports with good clinical results. 12,13 Two types of pupillary dilators commercially available are hooks and rings. The use of iris hooks for pupil expansion has gained worldwide popularity since their introduction in the early 1990s.…”

“…4 Iris manipulation during sphincterotomy triggers exudative transudation into the anterior chamber, thereby increasing the risk for uveitis exacerbation or reactivation. 14 In addition, limited surgical vision and multiple manipulations make removing Soemmerring's ring difficult compared with using a different type of pupil expansion ring. Iris retractors, such as iris hooks and Assia Pupil Expanders, expand the pupil to a rectangular or slightly trapezoidal shape of 6.0 mm or less to avoid iris tears.…”
