Zona pellucida (ZP) manipulation, termed "assisted hatching" (AH), has been introduced in order to favor embryo hatching and ultimately improve assisted reproductive technology success but with poor proofs of safety and biological plausibility. We herein provide a systematic review of clinical outcomes following the application of different methods of ZP manipulation on fresh or frozen/thawed embryos at different developmental stages in different groups of patients. Out of the 69 papers that compared the clinical outcomes deriving from hatched versus non-hatched embryos, only 11 considered blastocysts while the rest referred to cleavage stage embryos. The ZP thinning of fresh embryos either by chemical or laser approach was shown to provide very limited benefit in terms of clinical outcomes. Better results were observed with procedures implying a higher degree of zona manipulation, including zona removal. Studies comparing the mechanical or chemical procedures to those laser-mediated consistently reported a superiority of the latter ones over the former. Literature is consistent for a benefit of ZP breaching in thawed blastocysts. This review provides the current knowledge on the AH procedure in order to improve its efficacy in the appropriate context. Embryologists might benefit from the approaches presented herein in order to improve Assisted Reproduction Technologies (ART) outcomes.

ObjectiveTo identify differences in the expression of the genes for peroxisome proliferator-activated receptor (PPAR)-γ, cyclooxygenase (COX)-2, and the proinflammatory cytokines interleukin (IL)-6 and tumor necrosis factor (TNF)-α in granulosa cells (GCs) from polycystic ovary syndrome (PCOS) patients and controls undergoing controlled ovarian stimulation.MethodsNine patients with PCOS and six controls were enrolled in this study. On the day of oocyte retrieval, GCs were collected from pooled follicular fluid. Total mRNA was extracted from GCs. Reverse transcription was performed and gene expression levels were quantified by realtime quantitative polymerase chain reaction.ResultsThere were no significant differences in age, body mass index, and total gonadotropin dose, except for the ratio of luteinizing hormone to follicle-stimulating hormone between the PCOS and control groups. PPAR-γ and COX-2 mRNA was significantly downregulated in the GCs of PCOS women compared with controls (p=0.034 and p=0.018, respectively), but the expression of IL-6 and TNF-α mRNA did not show significant differences. No significant correlation was detected between the expression of these mRNA sequences and clinical characteristics, including the number of retrieved oocytes, oocyte maturity, cleavage, or the good embryo rate. Positive correlations were found among the PPAR-γ, COX-2, IL-6, and TNF-α mRNA levels.ConclusionOur data may provide novel clues regarding ovarian GC dysfunction in PCOS, and indirectly provide evidence that the effect of PPAR-γ agonists in PCOS might result from alterations in the ovarian follicular environment. Further studies with a larger sample size are required to confirm these proposals.

Gestation is the main goal for <em>in vitro</em> fertilization. The embryo that has been developed outside the body will be transferred directly into uterus leading to the process of hatching, implantation, and pregnancy. However, approximately 85% of embryos that have been transferred were failed to implant and it might be caused by hatching failure. Hatching is the process of releasing embryo from zona pellucida. If this process does not occur, it will cause pregnancy failure. Assisted hatching is a mechanism that dealing with thinning, slicing or artificially making holes in the zona pellucida to improve hatching. The process can be applied both in fresh or frozen embryos. This review describes various methods in assisted hatching such as enzymatic, chemical, mechanical, and laser beam as well as their advantages and disadvantages. Generally, some researches show that the technology of assisted hatching can improve the percentage of hatching and implantation of the embryo. However, in spite of the benefits, there are such weaknesses find in the zona pellucida of the embryo that has been manipulated such as toxic hazard medium, the risk of damage to the blastomeres or monozygotic twinning. Therefore, it is advisable to perform assisted hatching in certain cases that tends to face obstacles during the process of hatching such factors as age, embryo quality, the thickness of the zona pellucida and the number of failures in the <em>in vitro</em> fertilization program.
