In this study, we performed a network meta-analysis to compare the outcomes of seven most common surgical procedures to fix DRF, including bridging external fixation, non-bridging external fixation, K-wire fixation, plaster fixation, dorsal plating, volar plating, and dorsal and volar plating. Published studies were retrieved through PubMed, Embase and Cochrane Library databases. The database search terms used were the following keywords and MeSH terms: DRF, bridging external fixation, non-bridging external fixation, K-wire fixation, plaster fixation, dorsal plating, volar plating, and dorsal and volar plating. The network meta-analysis was performed to rank the probabilities of postoperative complication risks for the seven surgical modalities in DRF patients. This network meta-analysis included data obtained from a total of 19 RCTs. Our results revealed that compared to DRF patients treated with bridging external fixation, marked differences in pin-track infection (PTI) rate were found in patients treated with plaster fixation, volar plating, and dorsal and volar plating. Cluster analysis showed that plaster fixation is associated with the lowest probability of postoperative complication in DRF patients. Plaster fixation is associated with the lowest risk for postoperative complications in DRF patients, when compared to six other common DRF surgical methods examined.

Background Health and medical services are fundamental demand in civilized nations. Sanctions have always been an obstacle for development, even in health and medical topics, since they aim to reduce the financial and economic power of a country, and their impacts on medical and health systems in the objected country are on inevitable. It is well-established that sanctions contribute to a shortage of supplies in medical devices, and instruments. The shortage of the unique resources of newly-introduced medical and surgical devices prohibits surgeons from implementing novel techniques, which are known to improve medical care service quality. However, considering the application of the sanctions, these products will not be accessible for Iranian patients, and less safe and more complicated approaches will replace them. Results In this study we surveyed opinion of 32 orthopedic surgeons about effect of sanction on orthopedic surgery. Our result showed in upper limb surgeries high cost of equipment have more answers between question, although in lower limb surgeon Changing of method due to lack of facilities with worse results than standard method has more frequency between answer. Both results indicate that sanction made more difficult feasibility of orthopedic surgeon. Conclusions We believe several actions are needed to take place in the current situation by the international organization, to stop this unreasonable and illogical sanction, to prevent devastating results of them.

Background:Management of distal radius fractures (DRFs) is still controversial and may be influenced by the initial fracture classification. Even though numerous classification systems have been proposed in this regard, the evaluation and management of this fracture has remained problematic.Objectives:The purpose of this study was to evaluate the functional outcome of DRF managed on the basis of a new classification. This classification named as Barzullah Working Classification represents a modification of Melone classification, which is based on fracture stability.Patients and Methods:A total of 310 DRFs of patients skeletally matured referred to a tertiary care hospital at a period of 18 months were classified as per the new classification system into four types; metaphyseal stable, metaphyseal unstable, radiocarpal stable, and radiocarpal unstable fractures. They were managed and followed over a mean period of 15.10 ± 5.4 months, and the results were recorded at the final follow-up.Results:The mean age of the patients was 51.22 ± 20.58 years. Most of the patients were females (n=189, 64.19%). The minimal follow up was 6 months with a mean of 15.10 ± 5.4 months. Mean mayo wrist scores were 95 ± 4, 80 ± 7.4, 75 ± 7.4, and 70 ± 6.9, for stable metaphyseal fractures, unstable metaphyseal radial, stable radiocarpal fractures and unstable radiocarpal fractures, respectively. The overall mean mayo wrist functional score was 80.58 ± 12.3 (good results) at final follow up.Conclusions:Various modalities of treatment used differentially in different types of DRFs based on the Barzullah Working Classification give good results in spite of conflicting literature.
