Outcomes of AngioJet® Thrombectomy in Hemodialysis Vascular Access Grafts and Fistulas: Pearl I Registry

Abstract: Outcomes from the PEARL I registry corroborate other studies of AngioJet thrombectomy and the overall literature of endovascular thrombectomy procedures. A high procedural success rates with acceptable near-term patency rates and low rates of adverse events are achieved with AngioJet thrombectomy.

“…All interventions in hemodialysis patients should be planned and executed with the aim to preserve an individual patient's finite usable arterial and venous segments (access sites) for establishing future arteriovenous accesses. A number of percutaneous image-guided interventions are used in the management of dysfunctional hemodialysis access (AVG and AVF), including pharmacologic thrombolysis, mechanical thrombectomy, balloon angioplasty, stent or stent graft placement, or a combination of these methods (10)(11)(12)(13)(14)(15)(16)(17).…”

“…The outcomes might be hampered by the use of an older AngioJet™ system with lower power and lacking a standardized treatment protocol and expertise. More recent studies, including ours, were conducted with the newer, more powerful, AngioJet™ AVX system and show improved results, reporting clinical success rates of up to 97% …”

“…Endovascular mechanical thrombectomy using the AngioJet™ Peripheral Thrombectomy system (Boston Scientific, Natick, MA) can be deemed an effective technique, as described in a large multicenter registry for treatment of deep vein thrombosis . Experience with this system in occluded vascular access is relatively limited though, especially in native AVF, with varying clinical success and patency rates . Moreover, the quality of the evidence fluctuates …”
