Outcome of ulcer-associated duodenitis after short-term medical treatment

Abstract: The relationship between duodenitis and the outcome of duodenal ulcers was prospectively investigated by evaluating the frequency and extent of bulbar duodenitis before and after short-term medical treatment of the ulcer. Duodenitis appeared to be more frequent and more severe in the bulbar area close to the ulcer and was more widespread in the bulb of patients whose ulcer did not respond to treatment. After medical treatment, duodenitis improved only in the ulcer area and only in patients with complete ulcer … Show more

“…In contrast, open observations reported that cimetidine cannot influence the endo scopic features of ulcer-independent nonspe cific duodenitis [33], Although cimetidine was found to reduce somehow the severity of histological changes in duodenitis [36] and to decrease the amount of the inflammatory infiltrate [14] and of mucosal cell proliferation [35], the drug failed to prove superior to placebo in influencing the histopathological appearance [46,48],…”

“…Others have observed that after medical treatment for peptic ulcer only duodenitis confined to the ulcer area im proved and only in patients with complete reepithelization of the ulcer crater. In con trast, duodenitis located in areas far from the ulcer was not influenced either by the out come of duodenal ulceration or by antiulcer therapy [33].…”

“…For instance, Paoluzzi et al [26] showed that patients with complete healing after treatment of their DU had a 30% recurrence rate at 6 months, whereas patients with incomplete healing showed nearly 80% recurrence in the same period. In our prospective study [18,19] we found that during maintenance treatment with ranitidine, 150 mg, recurrence rates were twice as frequent in patients with erosive duodenitis as in those witli complete healing of all epithelial defects (23 and 12%.…”
