Twin pregnancy is assumed to increase the risk of maternal and neonatal complications. This situation is worse in Sub-Saharan Africa because of lack of well-equipped facilities. Studies on twin pregnancy and maternal complications are limited in Ethiopia. Thus, this study aimed to fill this gap. A hospital-based cohort study was conducted in Jimma University Specialized Hospital on 144 twin deliveries and 288 singleton deliveries. Data were collected through face-to-face interview by using structured-questionnaire and analysed by SPSS V.20.0. The relative risks of twin deliveries were significantly higher for the occurrence of hypertension, preterm labor, poor progress of labor, premature rupture of membrane, polyhydramnos, cord prolapse, uterine atony, postpartum haemorrhage, puerperal sepsis and maternal death as compared with singleton deliveries. In conclusion, women with twin pregnancy were at a higher risk of medical and obstetric complications during antepartum, intrapartum and postpartum periods. Thus, special emphasis should be given to mothers with twin pregnancies during antenatal care, delivery and postnatal care so as to prevent or manage the complications earlier.

Background: Incidence of twin pregnancy is increasing all over the world. It can occur after Assisted Reproductive Technology (ART) or spontaneously and associated with increased maternal and neonatal complications both in the developed and developing countries.Methods: A descriptive (Cross sectional) study of 64 cases of twins was undertaken between January 2013 till December 2015. Data collection was done prospectively from the patients admitted to the hospital with twin gestation. Inclusion criteria: All pregnancy with twin gestation more than 28 weeks of gestation. Twin pregnancies with both fetuses alive are included. Exclusion criteria is multiple gestation other than twins, cases with congenital malformation and intrauterine death (in one or both twins) were excluded. Data included thorough antenatal history, demographic details and intrapartum and postpartum maternal and neonatal details.Results: Prevalence of twins in our study was 1.61% (95% CI 1.3-2%). Out of 69 cases of twin pregnancy 64 cases fulfilled the inclusion criteria. History of ovulation induction was associated with 23% cases. Commonest complications observed were preterm labour (56.5%) anaemia (43.4%) and PIH (22.3%). 30.2% cases delivered vaginally whereas 69.8% patients required c. section, malpresentation being commonest indication. No intrapartum still birth was recorded. Early neonatal death was seen in eight cases, causes were prematurity, hyaline membrane disease and neonatal jaundice. Diamnoitic-dichorionic twins were 90% and 3% cases were monoamniotic monochorionic. Zygosity was calculated by Weinbergs formula and 84% cases were dizygotic while 16% cases were monozygotic. No maternal mortality related to twin pregnancy was reported in present study.Conclusions: All twin pregnancy should be considered as high risk pregnancies and should have mandatory hospital delivery. Early diagnosis, adequate antenatal, intrapartum and postpartum care as well as good NICU back up are the key factors in reducing the complications and improving the perinatal outcome in twin pregnancies.

Twin pregnancy carries higher foetal and neonatal complications. This situation is worse in Sub-Saharan Africa due to lack of well-equipped perinatal complications are limited in Ethiopia. Thus, this study aimed to fill this gap. A hospital-based cohort study was conducted in Jimma University Specialized Hospital on 144 twin deliveries and 288 singleton deliveries from December 2012 to November 2013 in obstetrics ward of Jimma university specialized hospital. Data were collected through face face interview by using structured-questionnaire and analyzed by SPSS V.20.0. The relative risks of twin deliveries were higher for the occurrence of low birth weight, need for neonatal intensive care unit admissions, early neonatal deaths and preterm births as compared with singleton deliveries. Unique foetal complications like both twin deaths, co discordant twin and gross congenital malformation were common in twin deliveries conclusion, women with twin pregnancy were at a higher risk of ante foetal and neonatal complications. Therefore, the mother during pregnancy, labour and delivery, and both for the mother and her new born during postpartum period.

Abstract: Considering that fetal prematurity and low birth weight, sequelae to preterm labor, are the commonest causes of perinatal death in this study, efforts should be geared during the antenatal period toward the prevention of a premature birth.
