Outcome of the Bone-Anchored Hearing Aid Procedure Without Skin Thinning

Abstract: The test group exhibited good preservation of the tissue, no increasing skin reactions and no adverse events. The time required for this surgery was reduced, as was their healing time. These patients also experienced less numbness and pain in the surrounding area and had an improved cosmetic outcome. MAIN OUTCOME AND CONCLUSION: This clinical trial indicates that introduction of the abutment to the osseointegrated screw directly through the skin, without skin thinning, could be beneficial. This approach had fe… Show more

“…Tissue preservation surgery, where the soft tissue is kept intact around the implant, was first reported in 2011. 12 Overall, the medium-to-long-term results of tissue preservation surgery are promising with skin reactions at least similar to control groups, and benefits including less numbness and more cosmetically pleasing outcomes being commonly reported. 1,3,9,10 This multicentre evaluation reported here demonstrates, short-term, similar positive outcomes for MIPS.…”

“…Prior to incision, skin thickness was measured to determine the appropriate abutment length according to the surgical manual 19, 20. Surgical techniques included the linear incision technique with soft‐tissue preservation6, 21 or the MIPS technique 6, 22. An elaborate description is provided in the protocol publication 6.…”

“…7 The first operation which used a tissue preservation technique was described in 2011, without skin flap and without removal of subcutaneous tissue. 3 This surgical technique is based on a new generation of abutments, whose main difference is their greater length in order to adapt to the thickness of the patient's skin and better integration with the subcutaneous tissue. 8 Although there are still a few published articles on this new technique, those that there are, describe very good results.…”

“…This modification avoids the removal of subcutaneous soft tissue and therefore the skin flap is less vascularly compromised. 3 The possible benefits of this technique would be fewer cutaneous reactions and complications.…”
