Outcome of Surgical or Endovascular Treatment of Giant Intracranial Aneurysms, with Emphasis on Age, Aneurysm Location, and Unruptured Aneuryms - A Systematic Review and Meta-Analysis

Abstract: BACKGROUND Designing treatment strategies for unruptured giant intracranial aneurysms (GIA) is difficult as evidence of large clinical trials is lacking. We examined the outcome following surgical or endovascular GIA treatment focusing on patient age, GIA location and unruptured GIA. METHODS Medline and Embase were searched for studies reporting on GIA treatment outcome published after January 2000. We calculated the proportion of good outcome (PGO) for all included GIA and for unruptured GIA by meta-analysis … Show more

“…Flow diverter stents now have expanded indications, including smaller aneurysms, and aneurysm up to the internal carotid artery bifurcation. Recently, a multicenter group published a retrospective study of follow-ups after PED placement [66], in this report overall complications were 3.4% and in multivariate analysis older age > 70, larger diameter > 15 mm and fusiform were identified as independent variables with higher rates of incomplete occlusion in 6-month follow-up. However, currently there is not a standardized scale to report Endovascular Treatment of Brain Aneurysms DOI: http://dx.doi.org /10.5772/intechopen.88964 radiographic outcomes that can be useful to meta-analysis studies or to new prospective randomized cohorts.…”

“…Peripheral injection of apelin-13 induced anxiolytic activity in a mouse model of chronic normobaric hypoxia by suppressing the nuclear factor κB (NF-κB) pathway, and silent mating type information regulation 2 homolog 1 (SIRT1) was involved in this process ( Fan et al, 2017 ; Fan et al, 2018 ). In addition, the apelin/APJ system had a protective effect on memory impairment ( Li et al, 2016 ; Haghparast et al, 2018 ), ischemic stroke ( Yang et al, 2016b ; Wu et al, 2018 ), and brain damage ( Bao et al, 2015 ; Chu et al, 2016 ). However, the role of apelin in inflammatory pain is still unclear.…”

“…Cerebral aneurysms are defined as giant when their diameter is ≥25 mm [1][2][3][4][5][6]. They represent <5% of all intracranial aneurysms, making them rare among all aneurysms [3][4][5]7].…”

“…They represent <5% of all intracranial aneurysms, making them rare among all aneurysms [3][4][5]7]. The natural history of giant intracranial aneurysms (GIAs) is characterized by progressive growth, thrombosis, and rupture [1,3,5,6,8,9]. Rarely, GIAs present with hemorrhage; therefore, many are revealed by neuroimaging performed for headache, seizure, stroke, or progressive focal deficit [7].…”
