Outcome of radioiodine‐131 therapy in hyperfunctioning thyroid nodules: a 20 years’ retrospective study

Abstract: After 20 years of follow-up, 60% of patients treated with 131I for a single hyperfunctioning nodule are hypothyroid. Factors increasing the risk of hypothyroidism are age, 131I uptake and MMI pretreatment. The prognostic value of this last factor, however, depends on the degree of suppression of the extranodular thyroid parenchyma at the scan.

“…Medical therapy by corticosteroids in the early stages of disease is useful to reduce the inflammation and to obtain GO remission, but it is useless in fibrotic endstages. Early diagnosis and treatment would therefore lead to at least partial remission in up to 65% of cases 22 . The CAS scoring system evaluates disease activity and response to immunosuppressive therapy.…”

“…A delivered activity of 3-8 MBq (80-220 mCi)/g of thyroid tissue has been used (15,16,37,38), although 3 MBq/g is rather low to have an acceptable cure rate if correctable hypothyroidism is the goal. Although some treating physicians choose to aim for a euthyroid state using an activity toward the lower end of this range, decreasing the administered therapeutic activity in an effort to achieve a euthyroid state can lead to prolongation of hyperthyroidism with adverse clinical sequelae.…”

“…For example, 550 MBq (15 mCi) may be used for the usual-size solitary nodule (;1.5-3 cm in diameter) and higher doses for larger nodules (37,38). The same approach could be applied to therapy of diffuse toxic goiter or Graves disease depending on the size and activity of the gland.…”

“…Radioiodine therapy normalizes thyroid function and significantly reduces thyroid volume. However, hypothyroidism often occurs, in up to 60 % of patients, several years after treatment [9].…”
