Актуальность. Среди всех болезней ревматологического профиля на долю ревматоидного артрита (РА) приходится 3 % случаев, причем численность таких больных растет. Для РА характерно системное поражение сосудов (ангиопатия), характеризующееся по международной Чэпел-Хиллской классификации как «васкулит, связанный с системным заболеванием». Цель работы: улучшить качество диагностики, установить новые звенья патогенеза и выделить прогностические критерии течения поражения сосудов при РА. Материалы и методы. Под наблюдением находился 131 больной РА. Соотношение мужчин и женщин составило 1 : 2, минимальная, умеренная и высокая степени активности заболевания — соответственно 1 : 2 : 1, средний возраст обследованных пациентов составил 45,70 ± 1,02 года, длительность клинической манифестации — 9,40 ± 0,68 года, I, II, III и IV стадии диагностированы у 8, 40, 34 и 19 % от числа больных соответственно. Выполняли эхокардиографию, сонографию и ультразвуковую допплерографию сосудов, биомикроскопию конъюнктивы, морфологическое исследование нефробиоптатов, определяли интегральные индексы клинической и инструментальной сосудистой патологии. Результаты. Системная ангиопатия наблюдалась у 61 % от числа больных РА, чаще в случаях высокой степени активности при наличии остеопороза, при этом развитие васкулита кожи и периферической вазонейропатии было тесно связано с уровнем в сыворотке крови антител к циклическому цитруллиновому пептиду, который, наряду с концентрацией С-реактивного протеина, обладает негативной прогностической значимостью в отношении сосудистой патологии. Появление дигитального артериита определяется активностью суставного синдрома, гломерулонефрита — высоким содержанием в крови циркулирующих иммунных комплексов, а наличие ангиопатии отражает повышение давления в малом круге кровообращения. Выводы. У больных РА развивается мезангиопролиферативный или мезангиокапиллярный гломерулонефрит в соотношении 2 : 1 с закономерным тубулоинтерстициальным компонентом и депозицией иммуноглобулинов и компонентов комплемента (в строме → клубочках → канальцах → сосудах), при этом структурные изменения сосудов почек тесно связаны с клинико-инструментальными проявлениями системной ревматоидной ангиопатии. Показатели в крови C-реактивного протеина более 25 мг/л и противоцитруллиновых антител более 40 Е/мл являются прогнознегативными критериями в отношении системной сосудистой патологии и повреждений стромы почек, а тяжелые изменения почечных канальцев относятся к факторам риска высоких темпов прогрессирования суставного синдрома.

Patients with autoimmune diseases (ADs) are a challenge for the intensivist; it is hard to differentiate among infection, disease activity, and combinations of both, leading to high mortality. This study is a retrospective analysis of 124 critically ill patients admitted to the intensive care unit (ICU) in a university hospital between 2008 and 2016. Bivariate case–control analysis was performed, using patients who died as cases; later, analysis using a logistic regression model with variables that were associated with mortality was conducted. Four variables were consistently associated with mortality in the logistic regression model and had adequate prediction value (Hosmer and Lemeshow statistic = 0.760; Nagelkerke R-squared = 0.494). The risk of death was found to be statistically associated with the following: shock at admission to ICU [odds ratio (OR): 7.56; 95% confidence interval (CI): 1.78–31.97, p = 0.006], hemoglobin level <8 g/dL (OR: 16.12; 95% CI: 3.35–77.52, p = 0.001), use of cytostatic agents prior to admission to the ICU (OR: 8.71; 95% CI: 1.23–61.5, p = 0.03), and low levels ofcomplement C3 (OR: 5.23; 95% CI: 1.28–21.35, p = 0.02). These variables can guide clinicians in the early identification of patients with AD with increased risk of death during hospitalization, leading to initial therapies seeking to improve survival. These results should be evaluated prospectively in future studies to establish their predictive power.

The purpose and objectives of this work were to study the nature of the heart, lungs and kidneys angiopathy in rats with a model of systemic autoimmune disease, carrying out comparisons of the results with extravasal morphological manifestations of the pathological process, the state of vascular endothelial function and activity of pro-inflammatory cytokines in the blood. The experiment was conducted on 40 non-linear rats. To simulate the disease animals were injected by complete Freund's adjuvant, a solution of splenic deoxyribonucleic acid cattle, mercaptopurine, methyluracilum and with food rats constantly received sulfate cadmium, lithium hydroxybutyrate and ammonium molybdate. Within two months from the start of the study animals were taken out of the experiment on the background of intraperitoneal nembutal anesthesia. Histological sections of the heart, lung and kidney tissue were stained with hematoxylin -eosin, alcian blue and by van-Gieson, becoming the PAS-reaction. In the animals with the proposed experimental model of systemic autoimmune disease observed morphological characteristics of lesions of the heart blood vessels, lungs and kidneys, the nature of the manifestations of which were interconnected with each other, which confirms the common pathogenetic angiopathy constructions at various diseases in clinical practice. In the genesis of cardiac, pulmonary and renal vessels lesions act the activation of pro-inflammatory cytokines and violation of vascular endothelial function, which has a certain practical significance. In clinical practice, not only purely systemic vasculitis, and other autoimmune diseases should be treated as angiopathy.
