Outcome of Mitral Valve Repair or Replacement: A Comparison by Propensity Score Analysis

Abstract: Background-There are no randomized trials comparing outcomes after mitral valve (MV) repair and replacement.Propensity scoring is a powerful tool that has the potential to reduce selection bias in nonrandomized studies. Methods-From the BC Cardiac Registries, 2 060 patients presented for MV surgery, with or without CABG between 1991 and 2000. We then identified 322 MV repairs who were then matched by propensity score to an equal number of MV replacement patients. We compared survival and freedom from re-operat… Show more

“…The resultant deterministic model was used to generate a propensity-to-be-missing score, defined as the probability for each patient of having a missing viral load reading. 27,28 The Cox proportional hazard model for detectable viral load was controlled for the propensity score as a re-weighting adjustment factor. Data were analysed using Stata version 10.0 (StataCorp, College Station, United States of America).…”

“…Chordal preservation appears to represent a significant departure from the trend of worsening results after conventional MVR [15], yet analogous to results associated with mitral reconstruction [16,17] . Nevertheless retention of the posterior leaflet with anterior leaflet resection is the more customary method of MVR this is probably for fear of left ventricular outflow obstruction by papillary muscles and chordae tendineae of the anterior leaflet [18] as is described in some isolated case reports.…”

“…[73][74][75][76] Surgical repair of the valve tends to be successful in the following cases: degenerative mitral valve disease, annular dilation, chordal rupture, leaflet perforation due to endocarditis and papillary muscle dysfunction due to ischemia. It is less likely to be successful in older patients with calcified deformed valves, rheumatic heart disease or severe sub-valvular thickening.…”
