Outcome of intravenous recombinant tissue plasminogen activator for acute ischemic stroke in patients aged over 80 years

Abstract: Rt-PA therapy would appear to improve prognosis in the elderly with ischemic stroke. This suggests that age alone should no longer be a barrier to rt-PA therapy. Geriatr Gerontol Int 2016; 16: 843-849.

“…Some studies have investigated treatment with conventional intravenous thrombolysis in the elderly 17 , 22) , however, patients over 80 years of age were mostly excluded from trials, and no large-scale studies distinctly investigated age-specific outcomes of mechanical thrombectomy 4) . The aging population is more susceptible to decreased neuronal plasticity, considerably due to a lack of neurologic restoration, therefore, older infarction patients have poorer overall outcomes 5) .…”

“…We have read with interest the study by Mione et al, recently published in Geriatrics & Gerontology International. 1 This study heats the debate about the risk-benefit ratio of thrombolysis in older adults. Indeed, despite the fact that the randomized controlled study Third International Stroke Trial (IST-3) 2 and a meta-analysis 3 on Recombinant Tissue Plasminogen Activator (rt-PA) for acute stroke suggest that i.v.…”

“…The secondary outcomes were sICH (symptomatic intracranial hemorrhage) and mortality at seven days and 90 days. We used the de nitions of sICH in the following three studies: Safe Implementation of Thrombolysis in Stroke-Monitoring Study (SITS-MOST)(8), National Institute of Neurological Disorders and Stroke (NINDS) (9), and European Cooperative Acute Stroke Study (ECASS ) (10).…”
