Outcome of Cesarean Scar Pregnancies Diagnosed Sonographically in the First Trimester

Abstract: If a woman has a first-trimester diagnosis of a cesarean scar implantation pregnancy and embryonic cardiac activity is present, expectant management offers the possibility of delivering a live-born neonate (62.5% in our study) but carries a substantial likelihood of hysterectomy at delivery due to placenta accreta (37.5% in our study), whereas minimally invasive therapy that interrupts the pregnancy largely eliminates the need for hysterectomy.

“…Live births resulting from a cesarean scar ectopic implantation have been described; however, these deliveries are frequently associated with hemorrhage and emergent cesarean hysterectomy [9, 181, 182]. In a series of 10 patients with cesarean scar EPs with fetal cardiac activity who elected for expectant management, 4 patients (40 %) had live births, 3 of whom (75 %) required hysterectomies; overall, 80 % required hysterectomies [183].…”

“…In view of this risk, there is a widely held view that early treatment of CSP is justified in order to prevent serious maternal morbidity associated with advanced CSP. Although the majority of women diagnosed with CSP are offered some form of treatment, there is no consensus on the optimal management strategy. Surgical evacuation of pregnancy can be performed transcervically or transabdominally.…”
