Outcome after olecranon fracture repair: Does construct type matter?

Abstract: TBW and PF of olecranon fractures had similarly excellent functional outcomes in this study. Patients undergoing PF had a longer time to union and slightly worse extension at final follow-up. TBW remains an effective treatment for appropriately selected olecranon fractures and in this cohort outperformed plate osteosynthesis.

“…These complication rates are similar to those reported in olecranon osteotomy in dogs, 29,30 and are also consistent with human findings, in which complications occurred in up to 92 and 20% of patients after TBW and PO, respectively. 7,9,10,13,18,24,[31][32][33][34] In our study, complications occurred more commonly within the first 2 months after TBW but appeared to be scattered over time after PO. Interestingly, failure of fixation requiring implant removal and alternative fixation was only observed after TBW, occurring in 18% of cases within the first month of surgery.…”

“…Although there are studies in the literature with similar results regarding the deficit of range of motion after operative treatment, even in the general population. 32 , 45 The results of these early studies regarding the non-operative treatment of displaced olecranon fractures in the elderly, while encouraging, are limited. Most of them are retrospective, with small numbers of patients and short-term follow-up.…”

“…Hard ware irritation is often reported with plate fixation and hardware removal rates are reported to be 0% to 20% in the literature [13] , [14] , [15] . Painful hardware is more frequent in tension-band wiring than plate fixation and our literature search revealed rates to be from 9% to 91% for the tension band technique 16 , [17] , [18] , [19] , [20] , [21] , [22] .…”
