Введение. Лечение пациентов с высокоэнергетическими травмами костей конечностей является трудной задачей. Цель. Представлен случай этапного лечения пациента с огнестрельным переломом костей голени. Материалы и методы. Первым этапом выполнена стабилизация перелома внешним фиксатором. Второй этап направлен на купирование инфекционного процесса-резекция некротических тканей, установка антибактериального спейсера, замещение дефекта мягких тканей. Третий этап-реконструктивная операция для замещения костного дефекта и консолидации костей голени. Результаты и обсуждение. Сформированная мембрана Масколе, компрессионный остеосинтез по Илизарову и несвободная аутопластика фрагментом малоберцовой кости позволили восстановить опорность голени в достаточно короткий срок. Заключение. Комбинация стимулирующих эффектов различных хирургических техник позволяет получить хороший результат в тяжёлых клинических ситуациях. Ключевые слова: дефект кости, метод Илизарова, аутотрансплантат, мембрана Масколе Introduction High-energy tibial fractures are challenging injuries to treat. Objective We report a case of a tibial gunshot fracture treated at several stages. Material and methods The fracture was first stabilized with external fixation device. The second stage aimed at the arrest of infection consisted of excision of necrotic tissues, placement of antibacterial spacer and repair of compromised soft tissues. Reconstructive surgery was produced at the third stage of treatment to address bone defect and provide consolidation. Results and discussion The Masquelet induced membrane technique, compression osteosynthesis with the Ilizarov frame and non-free autograft using fibular fragment facilitated recovery of supporting tibia functions within a relatively short period of time. Conclusion The combination of stimulating effects from different surgical techniques is useful to ensure a good outcome in a severe clinical case scenario.

реферат Цель исследования -дать определение и толкование «ишемического» дистракционного регенерата, формирующегося при компрометированном течении дистракционного остеогенеза и оценить эффективность механического воздействия на регенерат у пациентов с костными дефектами и ложными суставами. Материал и методы. Были успешно пролечены 17 пациентов с дефектами длинных костей предплечья и голени, имевших на этапах чрескостного остеосинтеза осложнение в виде нарушения дистракционного остеогенеза с формированием «ишемического» регенерата. средняя величина дефектов составила 22,3% (у пациентов с дефектами костей предплечья) и 20% (у пациентов с дефектами костей голени) по отношению к контралатеральному сегменту. при лечении пациентов применяли механическую стимуляцию костеобразования посредством компрессии и компактизации проблемных дистракционных регенератов c использованием двух методик. в I группе пациентов выполняли дополнительную остеотомию удлиняемого отломка. во II группе компактизировали регенераты на высоту соединительнотканной прослойки до контакта костных отделов регенерата. в работе были использованы методы описательной статистики. Результаты. у всех пациентов в результате механического воздействия на зоны компрометированного течения дистракционного остеогенеза удалось восстановить процесс новообразования костной ткани, претерпевшей в дальнейшем полную органотипическую перестройку. Заключение. на основании клинико-рентгенологических признаков компрометированного течения дистракционного остеогенеза предложено определение и толкование понятия «ишемический регенерат». выполненный ретроспективный анализ результатов механического воздействия посредством компрессии и компактизации компрометированных дистракционных регенератов без смены технологии остеосинтеза свидетельствует о его эффективности.Ключевые слова: «ишемический» регенерат, чрескостный остеосинтез, костные дефекты. abstractThe purpose of the study was to define «ischemic» distraction regeneration which happens during the compromised course of distraction osteogenesis and to show the effectiveness of the mechanical action on such regenerates in patients with bone defects and pseudarthrosis. Objective The aim of this study is to assess the clinical results of bifocal or trifocal bone transport using unilateral rail system in the treatment of large tibial defects caused by infection. Methods There were a total of 37 eligible patients with an average age of 40.11 ± 10.32 years (range, 18–57 years; 28 males and nine females) with large tibial defects due to infection who were admitted to our hospital from June 2006 to June 2016. Among the patients, 21 underwent bifocal bone transport (BF group), and the remaining 16 were treated with trifocal bone transport (TF group). The demographic data (age, sex, interval duration before bone transport, previous operation time), intraoperative outcomes (size and location of the defect, size of soft tissue defect), postoperative variables (lengthening speed, external fixation index, duration of regenerate consolidation and docking union), postoperative bone and functional outcomes evaluated by Association for the Study and Application of the Method of Ilizarov (ASAMI) scoring system, and postoperative complications evaluated by Paley classification (muscle contraction, axial deviation, delayed consolidation, pin problems, repeated fracture, joint stiffness and others) of the two groups were recorded and compared at a minimum follow‐up of 24 months. Results The mean duration of follow‐up after removal of fixator was 29.49 ± 4.34 months (range, 24–38 months). There was no statistically significant difference in the demographic data, intraoperative outcomes including size and location of the defect, size of soft tissue defect, as well as postoperative complications. However, postoperative functional result in the TF group were superior to those in the BF group at a minimum follow‐up of 24 months, and lengthening speed, external fixation index (EFI), duration of regenerate consolidation and docking union were significantly reduced in the TF group when compared with the BF group. Conclusions Treatment of large tibial defects caused by infection with trifocal bone transport using unilateral rail system could significantly improve postoperative functional recovery and reduce duration of regenerate consolidation and docking union. The present study provides novel insight for the treatment of large tibial defects caused by infection.
