Otitis Media, the Caregiving Environment, and Language and Cognitive Outcomes at 2 Years

Abstract: Both OME and hearing loss were more strongly related to the quality of home and child-care environments than to children's language and cognitive development. Study results might be explained either by suggesting that children in less responsive caregiving environments experience conditions that make them more likely to experience OME and/or by suggesting that it may be more difficult for caregivers to be responsive and stimulating with children with more OME.

“…26,[43][44][45][46][47][48][49] It should be noted, however, that children with cleft lip and palate go through far longer periods of sensory deprivation due to middle ear infection compared to children without this facial anomaly; thus, the cleft lip and palate population is different in this sense.…”

“…Third, Maw et al 37 did not adjust for important covariables, such as the educational level of the mother and IQ of the child. In their articles, as Roberts et al and Vernon-Feagans et al 16,30,34,36,46 suggested that it is important to account for any factors that might contribute to a child's language performance and, therefore, might interact with the treatment effect.…”

“…Three correlation studies 40,45,47 (Table 4) examined whether hearing loss from OME was associated with receptive language at age 1 to 2 years by using the SICD. Meta-analysis showed a statistically significant negative association between hearing loss and receptive language during infancy (Fig.…”

“…First, the methods to document OME differed among studies (Table 1); most studies used tympanometry, whereas some also used otoscopy. The age of OME documentation also differed and occurred during only the first year of life, 41,47 the first 2 years, 4,9,28,45 the first 3 years, 7,8,12,42,44,46 and after 2 years old. 6,43 Although there were variations in OME documentation, all documentation was prospective and done on multiple occasions.…”
