Cl eft lip and palate (CLP) is a risk indicator to middle ear alterations, which may damage the development of auditory abilities such as attention that is essential to learn new skills, oral and written communication. Studies on attention process with CLP population are recent and poorly explored in the specific literature. Thus, this study aims to contribute with new subsidies in the field as it investigates the performance of children with CLP in Sustained Auditory Attention Ability Test (SAAAT). Material and Method: Comparison of SAAAT performance between children with CLP and children without it. Prospective study. Results: ANOVA was used as variance analysis model with two factors to study the variables such as group and gender. The CLP group showed an average of 2.5 units higher than the control group. This difference is between 0.7 and 4.4 with 95% certain. Conclusion: children with cleft lip and palate had poorer performance on SAAAT when compared to those without such craniofacial anomaly, considering attention reduction only.

ABSTRACT. Objective. To study the effectiveness of ventilation tubes on the language development in infants with persistent otitis media with effusion (OME). All existing studies addressed children 3 years of age or older. Currently, OME is detected and treated with ventilation tubes at a younger age. Because of the critical relationship between age, hearing, and language development, we conducted a study of the effects of ventilation tubes on language development in infants 1 to 2 years old with persistent OME.Design. A multicenter, randomized, controlled trial (embedded in a cohort) with 2 treatment arms: 1) treatment with ventilation tubes (VT group; n ‫؍‬ 93); or 2) with a period of watchful waiting (WW group; n ‫؍‬ 94). Hearing loss and expressive and comprehensive language were assessed every 6 months, while tympanometry and otoscopy were performed every 3 months. Other factors with potential influence on language development were also included: adenoidectomy, hospital, attending day care, sex, age at randomization, educational level of the mother, upper respiratory infections, and the native country of the parents and older siblings. The trial was designed to allow for the detection of a mean difference in language development of 3 months or more between children allocated to the VT and WW groups.Results. No relevant differences were found in expressive or comprehensive language between the 2 groups after adjustment for educational level of the mother, IQ of the child, and differences at baseline.A principal component analysis showed that in the VT group, the children with frequent complaints improved 1.6 months more in comprehensive language than those with no or some complaints. The children with favorable language stimulation, however, did not improve more than the children with less favorable stimulation. No differences were found for expressive language among the various clusters. The probability to improve >3 months in comprehensive language was .48 (95% confidence interval [CI]: .29 -.68) for children with highly educated mothers versus .09 (95% CI: .02-.30) for children whose mothers had a low educational level. In the WW group, these changes were .30 (95% CI: .14 -.53) and .14 (95% CI: .04 -.35), respectively. The probability to improve >4 months in expressive language was .52 (95% CI: .32-.71) for children with highly educated mothers versus .06 (95% CI: .01-.31) for children whose mothers had a low educational level. In the WW group these changes were .42 (95% CI: .23-.64) and .11 (95% CI: .03-.35), respectively. In addition, there were delays in expressive language in both groups compared with their age expected values.The comprehensive language of the children who were effusion-free during the follow-up (n ‫؍‬ 54) improved 1.5 months (95% CI: ؊.2-3.2) more than that of the children who had persistent effusion during the entire follow-up (n ‫؍‬ 28). ABSTRACT. Objective. Considerable controversy surrounds whether a history of otitis media with effusion (OME) in early childhood causes later speech and language problems. We conducted a meta-analysis of prospective studies to determine: 1) whether a history of OME in early childhood is related to receptive language, expressive language, vocabulary, syntax, or speech development in children 1 to 5 years old and 2) whether hearing loss caused by otitis media in early childhood is related to children's receptive language or expressive language through 2 years of age.Methods. We searched online databases and bibliographies of OME studies and reviews for prospective or randomized clinical trials published between January 1966 and October 2002 that examined the relationship of OME or OME-associated hearing loss in early childhood to children's later speech and language development. The original search identified 38 studies, of which 14 had data suitable for calculating a pooled correlation coefficient (correlational studies) or standard difference between parallel groups (group studies). Random-effects meta-analysis was used to pool data when at least 3 studies had usable data for a particular outcome.Results. We performed 11 meta-analyses. There were no significant findings for the analyses of OME during early childhood versus receptive or expressive language during the preschool years in the correlation studies. Similarly, there were no significant findings for OME versus vocabulary, syntax, or speech during the preschool years. Conversely, there was a significant negative association between OME and preschoolers' receptive and expressive language (lower language) (0.24 and 0.25 standard difference, respectively) in the group studies. Additionally, hearing was also related to receptive and expressive language in infancy (3%-9% of variance).Conclusions. Our results indicate no to very small negative associations of OME and associated hearing loss to children's later speech and language development. These findings may overestimate the impact of OME on outcomes, because most studies did not adjust for known confounding variables (such as socioeconomic status) and excluded data not suitable for statistical pooling, especially from methodologically sound studies. Although some OME language differences were detectable by meta-analysis due to increased statistical power, the clinical relevance for otherwise healthy children is uncertain.
