AimTo analyse the effect of leucocyte‐ and platelet‐rich fibrin (L‐PRF) on bone regeneration procedures and osseointegration.Materials and MethodsAn electronic and hand search was conducted in three databases (MEDLINE, EMBASE and Cochrane). Only randomized clinical trials, written in English where L‐PRF was applied in bone regeneration and implant procedures, were selected. No follow‐up restrictions were applied.ResultsA total of 14 articles were included and processed. Three subgroups were created depending on the application: sinus floor elevation (SFE), alveolar ridge preservation and implant therapy. In SFE, for a lateral window as well as for the trans‐alveolar technique, histologically faster bone healing was reported when L‐PRF was added to most common xenografts. L‐PRF alone improved the preservation of the alveolar width, resulting in less buccal bone resorption compared to natural healing. In implant therapy, better implant stability over time and less marginal bone loss were observed when L‐PRF was applied. Meta‐analyses could not be performed due to the heterogeneity of the data.ConclusionsDespite the lack of strong evidence found in this systematic review, L‐PRF might have a positive effect on bone regeneration and osseointegration.

In oral surgery, there is an increased concern for soft and hard tissue wound healing processes, and the development of bioactive additives for targeted surgical sites has become an important challenge in the past three decades. Recently, platelet concentrates (PCs) have been identified as satisfactory bioactive materials that increase the speed of the healing process in peri-implant surgical sites. Moreover, recent convincing results in several clinical studies and literature reviews have demonstrated the importance of these bioactive materials in the stimulation of the healing process and have provided promising results for the use in the future. To stimulate and ensure the healing for both soft and hard tissues in the oral region, there is convincing evidence that PCs can serve as an autologous source of growth factors and healing cytokine biomolecules, such as platelet-rich plasma, platelet-poor plasma, and platelet-rich fibrin (PRF) release, which plays a crucial role in promoting hemostasis and the wound healing process. In the recent studies, the primary concern has been the PCs in general, and particularly, PRF. The following review attempts to discuss the current data for researchers and clinicians to understand the value of combining biological additives with platelet-derived products for the healing of surgical sites. This approach is of particular concern, as the critical processes and effect on the speed of action are a controversial topic for both researchers and clinicians alike.

(J Korean Assoc Oral Maxillofac Surg 2012;38:85-9) Objectives: This study was to evaluate the effect of vertical bone gain and success rate and analyze the failure cases using the hydrodynamic piezoelectric internal sinus elevation (HPISE) technique. Materials and Methods: Patients who had been operated in the three centers including Daegu Catholic University Medical Center were selected for this study. The mucoperiosteal flap was elevated, and the sinus floor was then broken by specially designed piezoelectric insert, with hydraulic pressure applied to the sinus membrane for even elevation. Afterward, implants were placed. Panoramic radiogram or computed tomogram was taken before and after surgery and at the second operation and prosthesis placement. Later, changes in vertical height were measured and compared. The survival rate was based on the criteria of Buser et al. and Cochran et al. Results: In this study, 8 implants failed out of a total of 169 implants, resulting a success rate of 95.3%. These failure cases were due to insufficient initial stability or sinus membrane perforation. The mean of radiographic vertical height change at prosthesis placement was 5.7 mm (0.5-10.5 mm). Conclusion: In this study, HPISE technique was found to be a predictable treatment for atrophic maxilla and an alternative technique to the lateral approach.
