Aim: The aim of this prospective study was to provide an estimation of the 10-year probability of the four major osteoporotic fractures using fracture risk evaluation scale (FRAX) and to review the risk factors for osteoporosis in males and females aged over 50 years in Erzurum, Turkey. Material and Method: An evaluation was made 1254 patients who presented at the Outpatient Clinics of State Hospital in 2013. Taking the risk factors and bone mineral density (BMD) values into consideration, and using the FRAX risk evaluation scale, the 10-year major osteoporotic (MO) and hip fracture risks were evaluated for all the participants. Results: While a significant difference was seen between the genders in respect of measurements of the osteoporosis (OP) major risk fracture with BMD determined as mean 7.4% in females and 5.77% in males, and without BMD, as 8.27% in females and 4.59% in males and without BMD, the hip fracture risk was 2.92% in females and 1.91% in males (p=0.016, p<0.001, p<0.001, respectively), no significant difference was determined in the hip fracture risk with BMD at 2.62% in females and 3.05% in males (p=0.517). With an increase in body mass index (BMI), there was a decrease in the OP risk (p<0.001). With BMD, the MO fracture risk groups were formed of 84.5% low risk, 10.5% moderate risk and 5% high risk. Discussion: If BMD values were taken as the basis for medical treatment in Turkey, the use of scanning tests and fracture risk evaluation scales such as FRAX would be useful. However, to be able to better determine the extent of OP and the fracture risk in the general population and maybe to be able to determine threshold values which may be different in our population, there is a need for further multi-center studies including greater numbers of patients. Early diagnosis of OP, identification of risk groups for early treatment and prevention, and determination of the risk before a fracture all play an important role in the prevention of morbidity and mortality associated with OP and in the reduction of healthcare costs. Therefore, the aim of this study was to determine cases of osteopenia and OP in the early stages by measuring BMD and to determine the 10-year fracture risk using the FRAX scale in patients presenting at our hospital. Keywords Material and MethodIn the study period of 2013, a total of 1254 patients aged over 50 years, with no previous OP treatment underwent a DEXA scan at State Hospital. BMD measurements were taken with a DEXA Hologic Q 2000 device of the posteroanterior lumbar spine (L1-L4) and hip (femoral neck). The machine calibrations, tests, controls, and phantom measurements were applied routinely by certified technicians. SummaryObjectives: In this study, we aimed to evaluate the effectiveness of Turkish FRAX model in the prediction of osteoporotic fracture risk and to determine the factors that may affect the results in this model. Material and Methods: Data of 104 patients with hip fracture who had underwent bone densitometry scan between 2009-2012, in Kastamonu and Afyon cities in Turkey, were assessed in this study. Patients were divided into 3 groups. Group-1; patients with osteoporotic hip fractures (n=36), Group-2; patients with other osteoporotic bone fractures (n=33), Group-3; patients with no osteoporotic fracture history (n=35). The recent osteoporotic fractures in patients were ruled out and by noting the former fractures and previously determined risk factors, fracture risks were calculated by Frax method. The groups were compared on the basis of fracture risks, t-scores and demographical characteristics. Results: A statistically significant difference was found between the patients with major osteoporotic fracture (MOF) in Group-1 and Group-2 and patients with no MOF history in Group-3 (p<0.05). When t-scores were compared between the groups, there was difference between Group-1 and Group-3 (p<0.05), there was no difference between Group-2 and 3 (p>0.05). With respect to average age in patients, a significant difference was found between Group-1 and 3(p<0.05), however, no statistical difference between Group-2 and Group-3 (p>0.05). Also, no statistically meaningful difference with regard to other risk factors was found between the groups (p>0.05). Conclusion: Results of this study: previous MOFs, advanced age and a remarkable decrease in t-scores are the main risk factors in osteoporotic fractures. Turkish FRAX ® model may be useful in the prediction of fracture risk in patients with increased risk factors in Turkish population. However, this study may not be adequate to rule out the concerns that Turkish Frax model must be revised to fulfill the expectations in determining osteoporotic fracture risk. Background: The aim of this paper was to evaluate the availability of FRAX for assessing osteoporosis risk, and to demonstrate the importance of vitamin D levels in COPD patients. Methods: Fourty-six males who fulfilled the COPD diagnostic criteria defined by GOLD were included. Age, race, BMI, physical activity frequency, smoking and dietary habits, age at COPD diagnosis, disease duration, fractures history, and medications use were determined. Levels of 25(OH)D were detected. BMD was measured by DXA at lumbar spine, femoral neck, and entire femur, and classified according to ISCD. FRAX score was calculated. Control group was composed of 40 non-smoker individuals without previous history of pulmonary diseases. Results: 25(OH)D levels were significantly different between patients and controls. In the COPD group, a statistically significant difference in vitamin D levels was detected among the A, B, C, and D grades, while no such significant differences in FRAX scores were detected. 25(OH)D levels were significantly low in COPD patients with disease exacerbations and hospitalizations in the previous one year. No correlation was detected between vitamin D levels and the FRAX score. A positive correlation was observed between vitamin D levels and T-score. FRAX scores were higher and vitamin D levels were lower in osteoporotic COPD patients than in non-osteoporotic COPD patients. Conclusion: Using FRAX for assessing osteoporosis in COPD can reduce fracture risk and allow adequate treatment. Since vitamin D levels are related to exacerbations and hospitalizations, vitamin D supplementation may be needed in COPD patients, especially in those with high FRAX scores.
