Osteoporotic hip fractures in non-elderly patients: relevance of associated co-morbidities

Abstract: Osteoporotic hip fractures (OHF) are not limited to elderly; however, studies in non-elderly are scarce. Thus, the aim of this study was to evaluate co-morbidities in non-elderly patients with OHF in a Community Teaching Hospital. All hospitalizations due to OHF during a 3-year period in a Community Teaching Hospital were retrospectively evaluated for co-morbidities, and patients 18-64 years old were compared with those ≥65 years old. Of all hospitalizations, 232 (0.73%) were due to hip fractures, and 120/232 … Show more

“…Relatively few studies regarding the effects of estrogens on bone loss in men have been conducted 9 . Chronic alcohol abusers have a higher risk for osteoporotic fractures compared to non-drinkers 37,38 . Findings from this study may be used to better understand the biological effects of GEN and to provide additional information regarding use of isoflavone supplements to reduce the risks of osteoporosis in men.…”

“…Given the association between older age and substance abuse with reduced bone mineral density (BMD) and osteoporosis in the general population [31,32,33], the choice of antiretroviral regimen in patients with HIV infection with additional risk factors for osteoporosis needs to be carefully considered. Although the rarity of these events precluded multivariate analysis in the current report, on univariate analysis, fractures occurred significantly less frequently with EFV/FTC/TDF versus EVG/COBI/FTC/TDF (IRR 0.47; 95% CI: 0.27, 0.81 and IRD −3.85 per 1000 person-years; 95% CI: −5.02, −2.78).…”

“…2 In fact, recent data suggests that diabetes may be the strongest independent risk factor for osteoporotic hip fracture in non-elderly patients 34 . Chronic hyperglycemia, hypoinsulinemia or insulin resistance, hypercalciuria, osteoblast dysfunction, inflammation, disruption of the growth hormone (GH): IGF-I: IGFBP system and diabetes pharmacotherapy may all contribute to this skeletal fragility.…”
