Background and objectives: Long-term peritoneal dialysis (PD) is accompanied by low-grade intraperitoneal inflammation, may eventually lead to peritoneal membrane injury with high solute transport rate and ultrafiltration failure. Osteopontin(OPN) is highly expressed with the pro-inflammatory cytokines stimulation in many cell types, and evolves in the process of tissue fibrosis. This study aimed to investigate the potential of OPN as a new indicator of peritoneal injury. Methods: We analyzed a total of 125 PD patients with end-stage renal disease, including 16 patients with continuous ambulatory PD(CAPD)-related peritonitis and 109 patients without peritonitis in a single renal center. The OPN levels in the overnight peritoneal effluents or in serum were analyzed using ELISA. In HMrSV5 cells, The OPN and fibronectin(FN) protein expression were identified using western blot analysis. Results: The OPN levels in overnight drained dialysate were significantly correlated with D/P Cr (P < 0.0001, R =0.54) and D/D0 glucose (P < 0.0001 R=-0.39). Logistical regression analysis showed that the OPN levels in peritoneal effluents was an independent predictive factor for the increased peritoneal solute transport rate (PSTR) (p < 0.001). The area under the receiver operating characteristic (ROC) curve of the OPN-PSTR model for identifying PSTR was 0.88, with 95% confidence interval (CI):0.81-0.95. The OPN was more abundant in peritoneal effluents of the CAPD-related peritonitis group compared with the patients without peritonitis (18.64±13.04 vs. 2.23±1.63 ng/ml, p<0.001). In the in vitro experiment, lipopolysaccharides(LPS) increased the OPN expression in HMrSV5 cells, whereas downregulation of OPN suppressed FN induction with transforming growth factor-β1(TGF-β1)stimulation. Conclusions: The OPN levels in drained dialysate were independently correlated with peritoneal transport status in accordance with the PET results. OPN was highly expressed in effluents in patients with CAPD-related peritonitis. Peritoneal mesothelial cells displayed a high expression of OPN under inflammatory stimuli and OPN was likely to be implicated in the progression of peritoneal fibrosis. Abstrak Perak sulfat merupakan garam anorganik dengan rumus Ag2SO4. Perak sulfat ini termasuk dalam senyawa ionic karena terdapat ikatan antara logam perak dengan spesi non logam sulfat. Perak sulfat memiliki. berat molekul yaitu 311,799 gr/mol dengan tampilan kristal tidak berwarna dan tidak mudah terbakar. Senyawa ini digunakan dalam penyepuhan perak (silver plating) dan pengganti tanpa noda untuk perak nitrat. Perak Sulfat mengalami interaksi molekul dalam pelarut Air. Tujuan penelitian ini adalah untuk mengetahui karakteristik molekul dan transpor ion Perak Sulfat. Metode yang digunakan dalam penelitian ini adalah penelusuran literatur dengan Endnote X7, pemodelan komputasi, dan perhitungan matematis. Data transpor ion Perak Sulfat yaitu dengan parameter konduktivitas, viskositas, mobilitas ion, dan daya hanyut menggunakan data hasil review beberapa jurnal penelitian yang berkaitan dengan Perak Sulfat sesuai tahapan pada fishbone. Secara termokimia, perak sulfat memiliki entalpi pembentukan standar, ΔfHo 298 -715 kJ/mol dan entropi molar standar, So298 16,74 kJ/mol. Kelarutan Perak Sulfat dalam air yaitu 0,79 gr/100 mL (20 o C) dan dapat larut juga dalam pelarut asam nitrat. Hasil penelitian konduktivitas larutan Ag2SO4 semakin meningkat seiring dengan kenaikan konsentrasi persen massa sesuai grafik hasil perhitungan. Energi optimasi MM2 Minimization, Optimasi MM2 Dynamics, dan Optimasi MM2 Properties berturut-turut yaitu 210. 2194 kcal/mol, 755. 377 kcal/mol, 216. 774 kcal/mol. Kecepatan hanyut molekul perak sulfat dengan potensial Ag2SO4 3,61 V dan jarak antara elektroda 1,5 cm yaitu 2, 407 V/cm. Kata kunci : Perak Sulfat, Interaksi molekul, Pemodelan I. Elevated Levels of Galectin-9 but Not Osteopontin in HIV and Tuberculosis Infections Indicate Their Roles in Detecting MTB Infection in HIV Infected Individuals

Galectin-9 (Gal-9) and osteopontin (OPN) play immunomodulatory roles in tuberculosis and HIV infections. Evaluation of their levels as well as their interplay with different pro-inflammatory cytokines is critical to understand their role in immunopathogenesis of HIV/tuberculosis co-infection considering the complexity of the disease. Plasma levels of these proteins were measured by ELISAs in HIV-negative individuals with pulmonary (n = 21), extrapulmonary (n = 33), and latent tuberculosis (n = 22) and in HIV infected patients with pulmonary (n = 14), latent tuberculosis (n = 17), and without tuberculosis (n = 41). Levels of pro-inflammatory cytokines were estimated by Luminex assay. Receiver operated characteristic curve analysis was performed to evaluate discriminatory roles of these proteins. Spearman's correlation analysis was performed with the markers of HIV and tuberculosis disease progression to evaluate their immunopathogenic roles. Gal-9 and OPN levels were higher in HIV uninfected patients with active tuberculosis than with latent tuberculosis. Gal-9 but not OPN levels were higher in HIV infected patients with active tuberculosis than with latent tuberculosis. Area under curve for Galectin-9 was >0.9 in HIV/tuberculosis coinfection and extrapulmonary tuberculosis. OPN and IL-6 levels were higher in patients with severe chest X-ray grade indicating its association with severity of the disease and positively correlated with each other. Stronger positive and negative correlations of Gal-9 levels, respectively, with viral loads and CD4 cell counts in HIV infected patients were observed than OPN levels indicating their association with HIV disease progression. Thus, significantly elevated Gal-9 levels were reported for the first time in HIV/tuberculosis co-infection and extrapulmonary tuberculosis in our study than single infections with HIV and tuberculosis. The study indicated a need for further evaluation of monitoring role of Gal-9 for detection of developing tuberculosis in HIV infected individuals. The findings also indicated differential roles of Gal-9 and OPN in the pathogenesis of tuberculosis and HIV infections.
