Osteomyelitis and septic arthritis from infection with Burkholderia pseudomallei: A 20-year prospective melioidosis study from northern Australia

Abstract: Timely microbiological diagnosis and prompt treatment of melioidosis involving bone and/or joint with appropriate intravenous antibiotics is important, as is adequate surgical drainage and debridement where indicated. A subsequent protracted course of antibiotic eradication therapy is important to avoid relapse of disease.

“…The predomination of male gender among osteoarticular melioidosis was also reported in the literature about bone and soft-tissue melioidosis [1,10]. Another risk factor reported in this study was diabetes mellitus, which has been reportd to be the superior predisposing factor for melioidosis worldwide.…”

“…The reason behind the lowrate involvement of osteoarticular tissue in melioidosis might be attributed to the low blood supply to these locations comparing to other organs and knee joint is the most commonly affected joint. 27 Melioidosis also can be misdiagnosed as tuberculosis due to similarities in clinical and laboratory findings between the two conditions. Our case of spinal melioidosis had radiological findings in MRI spine similar to Pott's disease.…”

“…Neurological melioidosis is less common as described in a study from Australia. 27,28 There are technical difficulties in assessing the susceptibility of B. pseudomallei to co-trimoxazole. 29 More than half (6, 54.55%) of the cases showed cotrimoxazole resistant in vitro.…”

“…In our patient, the disease affected one knee joint followed by septicaemia. Though septic arthritis and osteomyelitis can be the primary manifestation of meliodosis, these may occur following dissemination of disease from infection elsewhere in the body also 9 . Kosuwon et al compared patients with septic arthritis caused by B. pseudomallei versus other pathogens, and reported that patients with any concomitant disease were 12 times more likely to be infected with Meliodosis 9 .…”
