Osteomyelitis and Gentamicin-PMMA Chains

Abstract: In chronic osteomyelitis, effective local antibiotic therapy after surgical debridement can be achieved by implantation of gentamicin-PMMA chains. The gentamicin concentrations at the site of infection are higher than that after systemic application of the same antibiotic. The appearance of this treatment is a particular aspect from a radiological point of view. Even if this is rare, these images deserve to be known when looking for osteomyelitis. We report a case of a 21-year-old man from Russia who had a bon… Show more

“…In this case, infection control was achieved via radical debridement and introduction of antibiotic-impregnated PMMA into the defect. The PMMA served to occupy the site of the bone defect and prevent brosis, maintain tibial stability and release antibiotics [10,11]. Although antibiotics are selected according to culture and sensitivity testing data, extensive debridement of infected sites often results in segmental defects and presents management challenges [12].…”

“…It was first reported by Klemm that bone cement cannot only conduct good mechanical support but also have a certain antibacterial effect. It has been widely used in the treatment of open fractures, osteomyelitis, and prosthesis / foreign body infection [73][74][75].…”

“…However, there are relatively few public reports on ALBC for DSWI. Xia et al [74] applied ALBC to the defect reconstruction of DSWI to solve the problem of DSWI and sternal instability and achieved a definite effect. However, it is not sure whether ALBC should be removed, or whether it will release cytotoxicity and inhibit local bone perfusion and bone remodeling, which needs further clinical research.…”
