Osteolipoma in the Forearm

Abstract: A case of left distal forearm and wrist osteolipoma in a 56 year old female is reported. The patient presented with a 3 year history of nontender left wrist mass. Radiographs demonstrated a lobulated mass of mixed low density and calcifications, not adjacent to and with no connection to underlying bone. Ultrasound showed a spheroid hyperechoic lesion with internal heterogeneity and rim of calcifications. Magnetic resonance imaging revealed a lesion with predominantly fat characteristics on T1 weighted and T2 w… Show more

“…No other markers or details on osteogenic differentiation of these cells were reported. However, there are numerous reports on lipoma tissue ossification observed in different parts of the body [8,16,18,19,60,61,62,63] and this phenomenon could be explained by the existence of stem cells in lipoma tissue that may differentiate into osteoblasts and chondrocytes.…”

“…Lipoma with metaplastic bone (also referred to as osteolipoma) is a rare variant of lipoma in which both mature adipose tissue and mature lamellar bone are present [1, 39, 40]. Patients usually present with a soft tissue mass, which may be painful [3944].…”

“…Masses usually show low to isointense T1 signal intensity, overall heterogeneously high T2 signal intensity (Fig. 14), and heterogeneous intense enhancement [40, 50]. There are no reliable imaging features which discriminate calcifying aponeurotic fibroma from soft tissue sarcoma (particularly synovial sarcoma and undifferentiated pleomorphic sarcoma) [48, 50].…”

“…1 In general, these radiological features described in the literature and demonstrated in this case are important to distinguish this tumor from more aggressive lesions containing fat, such as liposarcoma. 5 The definitive diagnosis of osteolipoma is made by histopathological examination, 6 as illustrated in the case presently described. The histological features of osteolipoma include predominantly mature adipose components with irregularly distributed mature lamellar bone tissue.…”
