Osteoid osteoma of the patella: a case report

Abstract: Though osteoid osteoma is a common primary benign lesion of the bones, intra-articular involvement is rare and poses diagnostic difficulties when it affects middle-aged patients. We present the case of a 51-year-old woman with a 2.5 year history of anterior knee pain that was misdiagnosed as osteochondritis dissecans. Radiological findings were absent, whereas MRI showed a well-circumscribed lesion. A local excisional biopsy was performed and microscopic appearance confirmed diagnosis.

“…It is typically a small lesion (less than 1,5 cm) composed by a central nidus of osteoid tissue surrounded by a rim of reactive sclerotic bone [8, 10]. Osteoid osteoma can occur everywhere in the skeleton both in the cortex and medulla [11]. However, it most commonly affects the lower extremity with 50% of all cases involving the femur or the tibia [12].…”

“…In addition to open and invasive techniques such as en bloc resection and Burr-Down technique, minimally invasive techniques as CT-guided percutaneous drilling are also used [8, 11, 24]. The surgical resection is difficult even under CT imaging because patella is a small bone, has lesser bone stock, and has a short distance between the anterior cortex and chondral surface [24].…”

“…This is probably due to the similarity in the ossification of the patella with that of the epiphysis of long bones [11] . There are reports of tumors like osteoid osteoma, osteoblastoma, spindle cell sarcoma, osteitisfibrosacystica, aneurysmal bone cyst and myelomatous infiltration arising from the patella [15][16][17][18][19][20] . Osteosarcoma is the most frequent primary malignant tumor of patella [9] .…”
