BackgroundTo assess the relationship between sport and osteoarthritis (OA), and specifically to determine whether previous participation, in terms of level (elite or non-elite), type of sport, intensity or previous injury, was associated with OA.MethodsThis systematic review was developed using PRISMA guidelines. Databases were searched (to May 2016). Narrative review and meta-analysis (with risk ratio (RR) and 95% CIs) approaches were undertaken where appropriate. Study quality was assessed using GRADE.Results46 studies were included. Narratively, 31 studies reported an increased risk of OA, with 19 demonstrating an increased risk in elite athletes. There was an increased risk after sports exposure (irrespective of type; RR 1.37; 95% CI 1.14 to 1.64; 21 studies). It remained uncertain whether there was a difference in risk of OA between elite and non-elite athletes (RR 1.37; 95% CI 0.84 to 2.22; 17 studies). The risk was higher in soccer (RR 1.42; 95% CI 1.14 to 1.77; 15 studies) but lower in runners (RR 0.86; 95% CI 0.53 to 1.41; 12 studies). 9 studies showed an association with the intensity of sport undertaken and OA. 5 studies demonstrated a higher prevalence of OA following meniscectomies and anterior cruciate ligament tears. Overall, the evidence was of GRADE ‘very low’ quality.ConclusionsThere was very low-quality evidence to support an increased relationship between sports participation and OA in elite participants. It is unclear whether there is a difference in risk between elite and non-elite participants with further prospective studies needed to evaluate this. Pooled findings suggested that significant injuries were associated with OA in soccer players.

Objective To systematically review the evidence for an increased risk of osteoarthritis in the hip, knee, hand, wrist, finger, ankle, foot, shoulder, neck, and spine related to diverse occupational activities of men and women and to examine dose‐response information related to the frequency, intensity, and duration of work exposures and the risk of osteoarthritis (OA). Methods Established guidelines for systematic reviews in occupational health and safety studies were followed. MEDLINE, Embase, CINAHL, and Cochrane Library were searched from inception to December 2017. Studies were reviewed for relevance, quality was appraised, and data were extracted and synthesized. Results Sixty‐nine studies from 23 countries yielded strong and moderate evidence for lifting, cumulative physical loads, full‐body vibration, and kneeling/squatting/bending as increasing the risks of developing OA in men and women. Strong and moderate evidence existed for no increased risk of OA related to sitting, standing, and walking (hip and knee OA), lifting and carrying (knee OA), climbing ladders (knee OA), driving (knee OA), and highly repetitive tasks (hand OA). Variability in dose‐response data resulted in an inability to synthesize these data. Conclusion Evidence points to the potential for OA occupational recommendations and practice considerations to be developed for women and men. However, research attention is needed to overcome deficits in the measurement and recall of specific work activities so that recommendations and practice considerations can provide the specificity needed to be adopted in workplaces.

A test of the New Coimbra method of recording entheseal changes as applied to the foot skeleton

Prior studies have indicated that entheseal changes throughout the skeleton are largely reflective of age and body mass with mixed results concerning activity patterns. To add to this debate, this study tests the effectiveness of employing the New Coimbra method to record entheseal changes of the calcaneus (Calcaneal tendon enthesis), hallucal metatarsal (Peroneus longus m. tendon, and Tibialis anterior m. tendon entheses), and fifth metatarsal (Peroneus brevis m. tendon enthesis) to understand past activity patterns. Presence and severity of entheseal changes were scored for 71 white adults from the William M. Bass Donated Skeletal Collection, and these scores were contrasted according to age, sex, stature, body mass (BMI), and occupation. Spearman's rank correlations show that age has the greatest impact on changes for the Calcaneal tendon enthesis, the Peroneus brevis m. tendon enthesis, and the Tibialis anterior m. tendon enthesis. For all of these entheses, entheseal scores increase as age increases. Stature is the largest contributing factor to changes in the Peroneus longus m. tendon enthesis, and this relationship is negative: entheseal scores decrease as stature increases. BMI has a limited effect on entheseal changes in the foot skeleton, and this relationship may instead be mediated by gait differences related to stature. No statistically significant relationships between entheseal changes and occupational standing ranks were found, although this may be related to the ways in which occupations were recorded and ranked. It is possible that statistically significant effects of physical activity patterns on the expression of entheseal changes could be documented using other measurements of activity patterns beyond occupational standing. These findings support prior studies that established age as a major contributor to entheseal changes but show that stature also contributes to enthesis formation in the foot.
