Orthotopic Kidney Transplant: a Valid Surgical Alternative for Complex Patients

Abstract: Kidney transplant is the best alternative of treatment for patients with end-stage renal disease (ESRD). At present, a significant part of patients admitted to waiting list are older and have previous transplants or severe vascular atheromatosis. In these cases, orthotopic kidney transplant (OKT) could be an option. The aim of the study is to present our results with this technique in terms of surgical steps, complications, and outcomes. Between January 1977 and August 2014, 1549 kidney transplants were perfor… Show more

“…Two recent studies have recommended this technique for patients with advanced atherosclerosis disease of the iliac arteries, bilaterally retained iliac fossae from previous kidney transplants, pelvic malformations, venous thrombosis, and obesity. 1,2 The studies additionally described that the orthotopic technique is challenging, with a slightly higher complication rate. Nevertheless, the present case reveals that an almost orthotopic position between the right common iliac artery and the patient's liver is feasible in highly complicated cases with one of these indications.…”

“…[1][2][3] It remains arguable whether this is caused by poor vascularization due to an above-average length of the transplanted ureter. Two retrospective studies stated that the length of the transplanted ureter does not affect postoperative urologic complications, but these studies do not mention a ureter length exceeding 12 cm.…”

“…[17] However, the procedure is technically demanding and associated with high surgical complication rates (66.7%) when performed infrequently. [18] 3. How should one approach the chronic haemodialysis patient in need of an FHC?…”

“… 2 Because of technical ease and success, the heterotopic renal transplantation (HRT) in the right or left iliac fossa is generally preferred over the orthotopic renal transplantation (ORT). 3 , 4 However, in selected patients where HRT is not possible, and so ORT was shown to be a desirable alternative. 3 …”
