There are more relevant criteria to judge success of programs than levels achieved on test scores, especially the use of USMLE as part of a screening process for potential neurologists. Clinical competence is not determined by the use of standardized testing.If we want to utilize tests of competence for programs and potential neurologists, let us use some measures of patient care needs, e.g., number of graduates working in underserved rural or inner-city hospitals and clinics, time spent providing teaching and providing care in underserved hospitals, and time spent in neurology education in medical community and public arena. In the article, there is an implication that problems of neurology training programs are in some way related to demands of indigent patient care; this is inaccurate. To improve the quality of neurologists trained, let us propose meaningful guidelines, not revert to outdated and useless markers.
