Organizing Healthcare for Optimal Acute Ischemic Stroke Treatment

Abstract: Stroke is a major health-care problem that represents a leading cause of death and also the top cause of disability in adulthood. In recent years there has been a significant paradigm shift in treatments for acute ischemic stroke to favor earlier reperfusion therapy, mainly using the systemic infusion of recombinant tissue plasminogen activator. Subsequent trials found that combining this treatment with endovascular therapy was effective in selected patients. The increased complexity of acute stroke treatments… Show more

“…In recent years, various modeling methods have emerged to study organization of acute stroke care before or substituting randomized clinical trials (RCTs). 68,69 Clearly, RCTs can put specific interventions to the test in a real-life care system. The RACECAT trial (Transfer to the Local Stroke Center Versus Direct Transfer to Endovascular Center of Acute Stroke Patients With Suspected Large Vessel Occlusion in the Catalan Territory) is one such RCT comparing the drip-and-ship and mothership models in Catalonia, Spain.…”

“…The severity of stroke can vary; in Europe it is the second most common cause of death 5 but even if the patient recovers, rehabilitation can take many months, or even years, and some individuals have to learn to live with permanent disabilities as a result of the stroke 2 . The best outcome for the patient is achieved if the blood flow is restored as promptly as possible 6 . European guidelines indicate that treatment for ischemic stroke should occur within the first 4.5 h of the onset of symptoms 3 .…”

“…It is thought that this may be related to the great heterogeneity that is present in the stroke-causing thrombi that could make a clot more, or less, susceptible to degradation by rtPA or affect the likelihood of successful retrieval. Understanding the thrombus composition and structure could lead to more targeted and effective treatments for stroke 6 .…”

“…CT angiography (CTA) of the head and neck establishes if there is a large vessel occlusion where endovascular intervention would be of use and further delineation of candidacy ensues [2]. A complete workup should entail a stroke center of excellence, ideally with an on-site neurology team, but this is not always possible in remote areas of the country and patient care should otherwise be optimized [9]. A transthoracic echocardiogram (TTE) with Doppler and bubbles assesses for an embryologic lack of closure in the cardiac membrane between the top two chambers of the heart or atria [10].…”
