“…However, the required technology and organizational structure of these two approaches to medical care differ in fundamental ways.19,20 EM requires relatively little technology and interaction between doctor and patient, whereas COT requires an array of accessible (portable) diagnostic and curative technology with extended interaction between doctor and patient.21 EM is concerned with a narrow slice of the patient's life (his or her presenting symptoms), whereas COT is concerned with the significance and interrelationships of symptoms, presented or not; the client's willingness to recognize these symptoms; and aspects of the patient's life that bear on successfully treating these symptoms. 22 A variety of factors may influence the extent to which an outreach modality reflects EM or COT, but the feasibility of these contrasting approaches in a given population largely depends on the client's motivation for treatment and morbidity.22 EM is efficient in populations where clients are motivated for a clearly defined treatment (e.g., trauma); COT becomes necessary when morbidity is less well defined and clients may be ambivalent about treatment.
