Oral water soluble contrast for malignant bowel obstruction

Abstract: There is insufficient evidence from RCTs to determine the place of OWSC in predicting which patients with inoperable MBO will respond with conservative treatment alone. There is also insufficient evidence from RCTs to determine the therapeutic effects and safety of OWSC in patients with malignant small bowel obstruction.

“…However, a meta-analysis from the Cochrane Database of Systematic Reviews reported that there is also insufficient evidence from randomized controlled trials (RCTs) to determine the therapeutic effects and safety of OWSC in patients with MBO. 70 But in fact, in this meta-analysis, only 1 study, by Lee et al, 71 met the standard of the research, and there were only 9 cases enrolled; the researchers themselves could not draw conclusions from the study. Research on OWSC in MBO is sparse; the lack of evidence does not mean that OWSC is ineffective but that more research is needed.…”

“…The use of WSCA is well‐established in SBO for diagnostic, prognostic and therapeutic purposes . The evidence base for use of WSCA has focused on adhesive SBO, although there is some preliminary evidence for a role in malignant bowel obstruction . In particular, trials have demonstrated that use of WSCA provides prognostic information to predict which patients with SBO will not resolve on conservative management , providing the trigger for surgical intervention in this group.…”

“…Radiologic testing, typically including a computed tomography scan, will play a large role in determining if an operation should be considered or only medical therapies will be implemented. While oral water‐soluble contrast may have benefit in the benign setting, there is no evidence that it is of benefit with an MBO . Total parenteral nutrition is controversial and there is no evidence for improved survival or quality of life .…”
