Purpose: To evaluate our results in the management of socket contraction. Design: Case seriesMethod: One hundred and eighty eight patients with contracted sockets were retrospectively analyzed. Reasons for eye removal, type of initial surgery, presence of an implant, time of onset of contraction, degree of contraction, additional pathologies accompanying contraction, type of surgery for the contracted socket, additional interventions, number of surgeries, and final status were recorded and evaluated.Results: There were 101 (53.72%) male and 87 (46.28%) female patients, with an age range between 1 and 78 years (mean 35.46 years). Mean follow-up was 45.6 ± 22.34 months (range 5-120 months). Trauma was the reason for eye removal in 80(42.55%) patients. Enucleation performed in 146(77.66%) patients was the most common initial surgery. An implant was absent in 143(76.06 %) patients. Time of onset of contraction was longer than 5 years in 56 (29.79%) patients. Moderate or severe contraction was recorded in 134(71.28%) patients. Totally 229 procedures were performed for socket surface expansion and 30.32% of patients required more than one surgery. 151(80.32%) patients had additional pathologies. At the final visit, 138 (73.4%) patients had a good or acceptable result. Conclusion:Contracted socket remains to be a challenging entity of oculoplastic surgery. It may develop at any time following removal of the eye. Additional pathologies frequently accompany the contracted socket. Even though wearing an artificial eye eventually becomes possible in the majority of cases, it is still hard for some patients even after a series of reconstructive interventions.

A 46-year-old woman presented with chemical injury in both eyes after the instillation of undefined eye-drops prescribed by quack. She had an atypical presentation in the form of bilateral severe necrosis of both lids and whole palpebral conjunctiva. Extensive debridement with conjunctival epitheliectomy, tenonplasty and amniotic membrane transplantation (AMT) was performed. Characteristic differences from typical chemical injuries were a sectorial involvement of ocular surface and keratinisation over distorted lid margins. Lid margin needed replacement by labial mucous membrane for the restoration of ocular surface. Ocular surface reconstruction with AMT in acute phase and mucous membrane grafting for involved lid margins in late phase achieves vision salvage and avoids late complications in drug-induced chemical injury.

Piterjium tedavisinde birçok tedavi yöntemi tanımlanmış olsa da piterjium cerrahi bir problem olmaya devam etmektedir. Konjonktival fornikste kısalma, semblefaron gelişimi, korneal melting varlığında ya da kornea santralini de içine alan ileri piterjiumlarda bu sorun daha da büyük olmaktadır. Nüks varlığında; komplikasyon görülme olasılığı artmakta ve tedavi başarı şansı düşmekte-dir.1 Tekrarlayan cerrahiler, limbal bölgedeki bariyer fonksiyonuna zarar vererek, fibroz doku proliferasyonunda artışı beraberinde getirmektedir. Tedavisinde çoğunlukla klasik cerrahi teknikler yetersiz kalmaktadır. Konjonktival otogreft tekniği kısa cerrahi süresi ve düşük nüks oranları nedeni ile tercih sebebidir. Amniyotik membran greftleri; antiinflamatuar, antianjiyogenik ve antiproteolitik aktivitesi sayesinde skar oluşumunun önüne geçebilmektedir. Konjonktival otogreft ve amniyotik membran transplantasyonunun kombine edildiği çalışmalarda iyi sonuçlar alınmaktadır. Ancak yeterli, sağ-lıklı konjonktivanın olmadığı hastalarda ek cerrahi yöntemler gerekmektedir. Bu durumda mukoza grefti kullanımı düşünülmelidir. Mukoza greftleri oral (bukkal, dudak, sert damak) ya da nazal böl-geden alınabilir. Classical surgical techniques are mostly insufficient for treatment. The technique of conjunctival autograft is preferred due to short surgery duration and low recurrence rate. Amniotic membrane grafts; may prevent the scar formation with their anti-inflammatory, anti-angiogenic and anti-proteolytic activity. In studies of amniotic membrane transplantation combined with conjunctival autograft are taken good results. If there is not enough healthy conjunctiva; additional surgical procedures are required. In this situation, use of mucosal graft should be considered. Mucosal grafts can be obtained from oral (buccal, lips, hard palate) or nasal region. In this study; we presented a combined lip mucosa and conjunctival autograft with amniotic membrane transplantation procedure, in a case of severe pterygium.
