Oral Glutamine in Preventing Treatment‐Related Mucositis in Adult Patients With Cancer

Abstract: In summary, the favorable efficacy and low toxicity of oral glutamine observed in clinical trials we reviewed provide a strong rationale for large randomized placebo-controlled studies to further evaluate its efficacy in preventing mucositis in patients with cancer receiving chemotherapy and/or radiation.

“…46 Similarly, oral or intravenous glutamine can reduce the severity of mucositis, but clinical evidence for its efficacy in preventing infections is lacking. 47,48 Without any specific interventions, mucosal damage starts to heal just before, or with the recovery of granulocytes in peripheral blood (PB), 49 as neutrophil recruitment to the site of mucosal injury may occur up to a week before recovery of neutrophils in PB. 50,51 Reconstitution of the innate cellular immune system…”

“…For examples, glutamine, a non-essential amino acid was widely studied for its potential protective effect on mucositis, dermatitis and oespahagitis [114][115][116][117] ; pentoxifyline, a methyl xanthine derivative, decreased the risk of radiation toxicity in lung [118] ; benzydamine could prevent and treat oral mucositis [119] ; and sulfasalazine was evaluated for its effects on radiotherapy-induced enteritis [120] . To date, there are still a limited number of chemical RAs clinically used to minimize the severity and duration of toxicities associated with radiotherapy.…”

“…β-glucan is a polissacarid complex found in foodstuffs such as mushrooms [11] and oats [12], and is able to modulate immunity, thus maximising interactive potential, both by stimulating the survival of probiotic cultures, and through antitumoral activity [13][14][15][16][17]. In addition, probiotics constitute a group of functional substances closely related to the reduction of the side effects of CT, such as diarrhoea, while contributing to improvements in the immune system by helping to modulate the micro-biotics of the intestines [18][19][20][21].…”

“…In relation to mucositis, glutamine stimulates cicatrisation by strengthening the immune system, thereby aiding the recuperation of oral mucus. In the case of diarrhoea, glutamine is able to regenerate the villi by producing improvements in intestinal function [19][20][21][22][23][24][25].…”
