The liver hormone hepcidin is the central regulator of systemic iron metabolism. Its increased expression in inflammatory states leads to hypoferremia and anemia. Elucidation of the mechanisms that up-regulate hepcidin during inflammation is essential for developing rational therapies for this anemia. Using mouse models of inflammatory bowel disease, we have shown previously that colitis-associated hepcidin induction is influenced by intestinal microbiota composition. Here we investigate how two commensal bacteria, Bifidobacterium longum and Bacteroides fragilis, representative members of the gut microbiota, affect hepcidin expression. We found that supernatants of a human macrophage cell line infected with either of the bacteria up-regulated hepcidin when added to a human hepatocyte cell line. This activity was abrogated by neutralization of IL-1␤. Moreover, purified IL-1␤ increased hepcidin expression when added to the hepatocyte line or primary human hepatocytes and when injected into mice. IL-1␤ activated the bone morphogenetic protein (BMP) signaling pathway in hepatocytes and in mouse liver, as indicated by increased phosphorylation of small mothers against decapentaplegic proteins. Activation of BMP signaling correlated with IL-1␤-induced expression of BMP2 in human hepatocytes and activin B in mouse liver. Treatment of hepatocytes with two different chemical inhibitors of BMP signaling or with a neutralizing antibody to BMP2 prevented IL-1␤-induced up-regulation of hepcidin. Our results clarify how commensal bacteria affect hepcidin expression and reveal a novel connection between IL-1␤ and activation of BMP signaling. They also suggest that there may be differences between mice and humans with respect to the mechanism by which IL-1␤ up-regulates hepcidin.Hepcidin is a 25-amino acid peptide hormone that is secreted by the liver and functions as the key regulator of systemic iron homeostasis (1). Its expression in hepatocytes is regulated exclusively at the level of transcription and is sensitive to a number of inputs, including tissue and plasma iron concentrations. Although the exact mechanism by which tissue iron concentrations are detected is not clear, it is known that elevated hepatic iron concentrations lead to increased expression of bone morphogenetic protein 6 (BMP6) (2). BMP6 acts on the BMP 3 receptor and its co-receptor hemojuvelin on the hepatocyte surface to activate signals that lead to phosphorylation of the receptor-associated proteins small mothers against decapentaplegic (SMAD) 1, 5, and 8, which then interact with SMAD4. The complex of SMAD4 and the phosphorylated receptor-associated SMAD translocates to the nucleus, binds to specific sites in the hepcidin promoter, and increases transcription (3, 4). Como objetivo secundario se analizó la relación del incremento de Hb con los parámetros utilizados para evaluar la anemia. Material y método: Estudio observacional retrospectivo realizado en 2 centros entre enero de 2005 y diciembre de 2009. Se incluyeron pacientes sometidos a cirugía ortopédica electiva con déficit de hierro y que fueron tratados con hierro sacarosa intravenosos solo o asociado a rHuEPO. Se analizó la efectividad del tratamiento mediante la diferencia entre la Hb basal y la Hb previa a la cirugía y se valoró la relación entre los parámetros hematimétricos y del metabolismo del hierro con la efectividad del tratamiento. Resultados: Se incluyeron 412 pacientes que recibieron una mediana de 800 mg de hierro sacarosa. A 125 pacientes (30,4%) se les asoció 2,4 viales de rHuEPO. El incremento de Hb fue de 0,8 (1,1) g/dL en los pacientes tratados con hierro intravenoso y de 1,5 (1,3) g/dL en los que se asoció rHuEPO (p < 0,01). El incremento de Hb en los pacientes tratados con hierro se correlacionó con el porcentaje de hematíes hipocromos (r = 0,52) y el valor del receptor soluble de la transferrina (r = 0,59). Results: A total of 412 patients who received a median of 800 mg of iron sucrose were included; 125 of them (30.4%) additionally received 2.4 vials of rHuEPO. The Hb increase was 0.8 (1.1) g/dL in patients treated with intravenous iron and 1.5 (1.3) g/dL in those additionally given rHuEPO(P < .01). The percentage of hypochromic red blood cells (r = 0.52) and soluble transferrin receptor (r = 0.59) value were significantly correlated to the Hb increase in patients receiving iron. Conclusions:In patients with iron deficiency anaemia, the effectiveness of iron sucrose treatment to optimize Hb before surgery was moderate; adjuvant administration of erythropoietin improved the results. Determination of functional iron status parameters may improve the treatment effectiveness.

INTRODUCCIÓNLa enfermedad inflamatoria intestinal (EII) se caracteriza por la afección del aparato digestivo y frecuentemente de otros órganos y sistemas. Por este motivo, podemos dividir el espectro sintomático de tal entidad en dos grandes grupos: la patología propiamente digestiva y las manifestaciones extraintestinales, dentro de las cuales se incluye de forma muy prevalente la anemia.Comparada con otras complicaciones, la anemia ha recibido históricamente una escasa atención por parte de los gastroenterólogos (1,2). El insuficiente control de los pacientes con dicha complicación ha conllevado un empeoramiento de su calidad de vida (3,4). Además, la falta de estrategias adecuadas en el tratamiento de la anemia ha tenido como consecuencia un aumento de la morbilidad asociada a esta entidad (5,6). Esto puede traducirse en un aumento de los requerimientos transfusionales de los pacientes, de la necesidad de ingreso para su tratamiento y del aumento de su estancia media hospitalaria.En la presente revisión se abordarán los aspectos más relevantes sobre etiología, diagnóstico y tratamiento de la anemia en la EII. Por otro lado, el carácter complejo de los mecanismos de producción de la anemia en la enfermedad inflamatoria intestinal con frecuencia plantea dudas acerca del tratamiento más adecuado. La identificación correcta de los pacientes con anemia así como la instauración del tratamiento más idóneo serán los dos pilares fundamentales para la mejoría de la calidad de vida de los enfermos.
