Cryptococcosis is a neglected fungal disease. The scarcity of studies on oral cryptococcosis is certainly due to rarity and/or underreporting of the disease, especially in Brazil. We describe an example of orofacial cryptococcosis affecting a 57-year-old man after heart transplantation, who presented with multiple erythematous ulcers and erosions distributed in the chin, nasal cavity, labial mucosa, hard palate, and buccal vestibule. Computed tomography revealed opacities and micronodules in the lungs. Histopathological features of the oral and pulmonary lesions were compatible with Cryptococcus spp. Amphotericin B and fluconazole were used for treatment during hospitalization and itraconazole for prolonged therapy after hospital discharge. The patient has been under follow up for 6 months without signs of disease. According to a review conducted in PubMed, Web of Science, Scopus, Embase, and LILACS for data analysis of oral cryptococcosis, 26 reports were described in the literature. Predilection for men was observed (85%), with a male:female ratio of 5.5:1. The mean age of the individuals was 49 ± 15.3 years. Oral cryptococcosis mostly presented as an ulcer ( n = 17). The palate and tongue were the most affected sites ( n = 9 for each). Amphotericin B was the primary therapy utilized in most patients. Seventeen (65%) individuals survived. Knowledge of the clinicodemographic aspects of oral cryptococcosis is important for clinicians in decision making and surveillance.

The frequency of fungal infections is increasing due to immunodeficiency viruses and immunosuppressive drugs. The most common fungal infection of the oral cavity is candidiasis. The existence of Candida can be a part of normal commensal; hence, the isolation of Candida in the absence of clinical symptoms should exclude candidiasis. The pathogenicity of Candida is witnessed as opportunistic when immune status is compromised. Oral fungal infections are uncommon, but when identified, these infections are associated with greater discomfort and are sometimes destruction of tissues. Cytology and tissue biopsy are helpful in confirming the clinical diagnosis. The management of oral fungal infections must strategically focus on signs, symptoms, and culture reports. This article reviews information on diagnosis and therapeutic management of aspergillosis, cryptococcosis, histoplasmosis, blastomycosis, mucormycosis, and geotrichosis.

As COVID-19 cases are reducing worldwide, the patients recovered from this disease are showing some complications include impact on lungs, kidneys, heart, and cases of a black fungal infection, called mucormycosis. There are a variety of bacterial and fungal co-infections which is associated with poorly controlled diabetes mellitus and other immune impaired condition. The COVID-19 exhibits, a specific pathophysiological feature, which will result in secondary fungal infections. The recent COVID-19 wave showed that many children and young people are testing positive for this disease. There is high chance for some of them can come across an orthodontic clinic. Oral mucormycosis is normally developed from the breathing of spores or through an open contaminated oral wound, affecting mainly immunocompromised patients. Oral mucormycosis is reported in some patient, following tooth extraction. The fungal spore can reach the body when there is a loss of anatomical continuity as can occur after a tooth extraction or in an ulcer. Chance for Post COVID-19 Mucormycosis in orthodontic patients are very rare. However, Orthodontist should take care when doing minor surgical procedure in the patient recovered from severe COVID-19 infection. Orthodontic appliances can increase the number of bacteria and fungus by limiting oral hygiene. Orthodontist should advise patients to the importance of maintaining basic oral hygiene measures, which include simple things like changing the toothbrush once they test negative, oral rinsing and brushing.
