ObjectiveThis study aims to explore the difficulties related to fresh colostrum feeding for very (extremely) low birth weight infants (VLBWI/ELBWI) and optimize the colostrum administration process.MethodsThe VLBWI/ELBWI who were admitted in the neonatal intensive care unit from January to December 2021, were enrolled as the experimental group, and an optimized colostrum feeding process was adopted. The VLBWI/ELBWI admitted from January to December 2020 were enrolled as the control group, and a conventional feeding process was adopted. The general situation of colostrum supply, number of adverse feeding events, maternal breastfeeding rate at the critical time points.ResultsThere were no significant differences between the baseline charatcteristics of the 2 groups. In the experimental group, compared with the control group, the time to first colostrum collection was significantly shorter (64.8% vs. 57.8% p < 0.05), and the rates of colostrum feeding (44.1% vs. 70.5% p < 0.001), and of maternal breastfeeding at 2 weeks after birth (56.1% vs. 46.7%, p < 0.05) and on the day of discharge (46.2% vs. 37.8%, p < 0.05) were significantly higher. Before and after process optimization, the average total time required for the time for nurses to receive the colostrum in the NICU reduced from 7.5 min/time to 2 min/time, and no feeding-related adverse events occurred.ConclusionFresh colostrum feeding process optimization for VLBWI/ELBWI, improves the colostrum feeding rate, shortens the time to first colostrum collection, saves the working time of nurses, and improves the maternal breastfeeding rate at key time points.

Objective To evaluate the effect of colostrum therapy on days to start a suckling diet in newborns diagnosed with simple gastroschisis. Methods Randomized clinical trial with newborns diagnosed with simple gastroschisis at a federal hospital in Rio de Janeiro who were randomized to receive oropharyngeal administration of 0.2mL of colostrum or a “sham procedure” during the first 3 days of life. The analysis included clinical outcomes such as days without food, days with parenteral feeding, days until the start of enteral feeding, days to reach complete enteral feeding, sepsis and length of hospital stay. Results The onset of oral feeding (suction) in patients with simple gastroschisis in both groups occurred at a median of 15 days. Conclusion The present study showed that there were no significant differences in the use of colostrum therapy and the number of days to the start of enteral feeding and suction diet between groups of newborns with simple gastroschisis.

Os primeiros 1.000 dias de vida compreendem desde o momento da concepção até os dois anos de idade da criança, sendo 270 dias da gestação mais 730 dias dos dois primeiros anos de vida. Também chamado de período de ouro, é considerado um período crucial para o crescimento e desenvolvimento infantil, pois trata-se de um período de 'janela de oportunidades', no qual é possível adotar hábitos e atitudes que irão influenciar o futuro do bebê. O presente trabalho buscou revisar na literatura estudos sobre condições nutricionais e práticas alimentares relacionadas aos principais agravos em saúde bucal que acometem gestantes e crianças nesse período de vida, além do impacto da odontopediatria nos primeiros mil dias. Foi realizada uma Revisão de Literatura Narrativa, com busca eletrônica nas principais base de dados: Scielo, Portal BVS, Pubmed e Google acadêmico. Foram utilizados os seguintes descritores: Nutrição infantil; Odontopediatria; Mil dias; Sistema estomatognático; Saúde materno-infantil (Child nutrition. Pediatric dentistry. thousand days. Stomatognathic system. Maternal and child health). Foram incluídos no estudo, artigos publicados em português e inglês entre os anos de 2015 e 2022. Artigos não relacionados ao tema proposto e publicados anteriormente ao prazo proposto foram excluídos. Foi observado que a odontopediatra tem papel importante, não apenas no âmbito curativo e da orientação de higiene bucal, mas também, na formação de hábitos saudáveis no pré e pós-natal. Existem evidências de associação entre condições nutricionais e práticas alimentares e sua relação com agravos bucais nos mil dias, mas não consolidado na literatura.
