Oral and Dental Considerations in Pediatric Leukemic Patient

Abstract: Throughout the world, there have been drastic decline in mortality rate in pediatric leukemic population due to early diagnosis and improvements in oncology treatment. The pediatric dentist plays an important role in the prevention, stabilization, and treatment of oral and dental problems that can compromise the child's health and quality of life during, and follow up of the cancer treatment. This manuscript discusses recommendations and promotes dental care of the pediatric leukemic patients.

“…Palifermin is one of the newest agents to be proposed to prevent the development of mucositis . It has been suspected that the decreased proliferative ability of oral epithelial cells has a role in the pathogenesis of mucositis; hence, several growth factors that can increase epithelial cell proliferation have been investigated for OM management . Palifermin is a recombinant keratinocyte growth factor (KGF) that is used to treat patients suffering from hematological malignancies undergoing myeloablative therapy, which is commonly associated with a high incidence of severe mucositis.…”

“… Frequent topical fluoride applications in the form of mouthwash or toothpaste and pit and fissure sealants on the molars prevent dental caries. The dentist can apply concentrated topical fluorides if necessary . The dental team should give dietary advice and inform the patient which type of foods should be avoided when possible: restriction of sticky food and dietary sucrose, elimination of food that irritate the oral mucosa. Sodium bicarbonate or saline mouthwash could be considered to reduce the frequency of oral mucositis and oral pathogens in children with ALL . The use of chlorhexidine rinses is controversial .…”
