Surgery for the morbidly obese is no longer a novelty. Understanding the basic mechanisms of the pathophysiology of morbid obesity and taking simple precautions while preparing these patients for surgery will help reduce complications. This review outlined preoperative, perioperative, and postoperative management of the morbidly obese patient in line with the current knowledge. Airway management, drug choices, changes in respiratory, circulatory, urinary, and metabolic systems were discussed. Some early and long-term changes in the morbidly obese, like gastroesophageal reflux and vitamin deficiencies were also summarized to aid in subsequent operations and management.

ÖZETPulmoner hipertansiyon, akciğer hastalıklarının ileri evrelerinde sıklıkla ortaya çıkar ve kötü prognoz göstergesidir. Daha önce sekonder pulmoner hipertansiyon olarak adlandırılan, akciğer hastalıklarına bağlı olarak ortaya çıkan pulmoner hipertansiyon, son dönemde yapılan pulmoner hipertansiyon sınıflamasında ayrı bir kategori olarak ele alınmıştır. Kronik obstrüktif akciğer hastalığı seyri sırasında saptanan pulmoner hipertansiyon, diğer akciğer hastalıkları seyrinde saptanan pulmoner hipertansiyona kıyasla insidansı ve klinik seyri en iyi bilinen gruptur. Akciğer hastalıklarının ileri dönemlerinde ortaya çıkan pulmoner hipertansiyonun patofizyolojisi ve tedavisi pulmoner arteriyel hipertansiyondan daha farklıdır. Bu grup pulmoner hipertansiyona yol açan majör mekanizmanın kronik hipoksemi olduğu bilinmektedir. Bu makalede akciğer hastalık-ları veya hipoksemi nedeniyle ortaya çıkan pulmoner hipertansiyon ile ilgili yeni bilgiler ele alınmıştır. ABS TRACTPulmonary hypertension often complicates the course of patients with advanced lung disease, and it is associated with a worse prognosis. In the recent classification of pulmonary hypertensive disorders, pulmonary hypertension due to lung disease is considered as a separate category within a group of disorders that were previously referred to as "secondary". The incidence and clinical course of pulmonary hypertension associated with chronic obstructive pulmonary disease is best known among the pulmonary hypertension associated with lung diseases. The pathophysiology and treatment of pulmonary hypertension seen in the course of advanced lung diseases are generally distinct from those of pulmonary arterial hypertension. Chronic hypoxia is the major mechanism implicated for the development of pulmonary hypertension in patients with lung diseases. Obesity currently affects about one third of the U.S. population, while another one third is overweight. The importance of obesity for certain conditions such as heart disease and type 2 diabetes is well appreciated. The effects of obesity on the respiratory system have received less attention and are the subject of this chapter. Obesity alters the static mechanic properties of the respiratory system leading to a reduction in the functional residual capacity (FRC) and the expiratory reserve volume (ERV). There is substantial variability in the effects of obesity on FRC and ERV, at least some of which is related to the location, rather than the total mass of adipose tissue. Obesity also results in airflow obstruction, which is only partially attributable to breathing at low lung volume, and can also promote airway hyperresponsiveness and asthma. Hypoxemia is common is obesity, and correlates well with FRC, as well as with measures of abdominal obesity. However, obese subjects are usually eucapnic, indicating that hypoventilation is not a common cause of their hypoxemia. Instead, hypoxemia results from ventilation perfusion mismatch caused by closure of dependent airways at FRC. Many obese subjects complain of dyspnea either at rest or during exertion, and the dyspnea score also correlates with reductions in FRC and ERV. Weight reduction should be encouraged in any symptomatic obese individual, since virtually all of the respiratory complications of obesity improve with even moderate weight loss.
