Optimization and repeatability of multipool chemical exchange saturation transfer MRI of the prostate at 3.0 T

Abstract: Background Chemical exchange saturation transfer (CEST) can potentially support cancer imaging with metabolically derived information. Multiparametric prostate MRI has improved diagnosis but may benefit from additional information to reduce the need for biopsies. Purpose To optimize an acquisition and postprocessing protocol for 3.0 T multipool CEST analysis of prostate data and evaluate the repeatability of the technique. Study Type Prospective. Subjects Five healthy volunteers (age range: 24–47 years; median… Show more

“…increased B 0 inhomogeneities and additional non‐rigid motion. ICC values were reported as 0.963 (95% confidence interval 0.852, 0.991) for conventional APT‐CEST in the breast and as 33‐38% LOA for conventional APT‐ and rNOE‐CEST in the prostate . In comparison with the values in this study (ICC = 0.902 with a 95% confidence interval of 0.888, 0.913; LOA = 16%), the reliability of dualCEST is equal or even better.…”

“…In contrast to using only a single region of interest (e.g. a whole organ or tissue), this “fixed window” approach allowed for a larger sample size for the statistical analyses. It should be kept in mind that in doing so the following assessment results are only valid for evaluating regions of 3 × 3 × 1 voxels.…”

“…In addition, these studies are using widely ranging evaluation methods and also investigating different entities, which is why a direct comparison has to be taken with caution. Only in the male prostate, relative LOA values evaluated in a complete ROI were reported ranging between 17 and 38% for the APT and rNOE signal 46 . In comparison to these values, the reproducibility of our protocol is equal or even better.…”

“…Four amine protons were considered for each creatine molecule. e For simulation, the concentrations were taken from references 52,53 , and exchange rate measured in vivo from rat brain. 4 Three amine protons were considered for each glutamate molecule.…”
