Optimising medication in frail older people

Abstract: Medicines optimisation is key to the successful management of frail older people in order to reduce harm and minimise inappropriate admissions. This article discusses the screening tools available to help identify frailty and gives guidance on how to optimise prescribing in frail individuals.
