Optimal Timing for Hemoglobin Concentration Determination after Total Knee Arthroplasty: Day 1 versus Day 2

Abstract: PurposePostoperative hemoglobin (Hb) determination remains an essential parameter for quantifying blood loss following total knee replacement (TKR) surgery and guiding transfusion practice. In this study we aimed to ascertain the optimal timing for Hb determination postoperatively and assess its relationship to serum hematocrit (Hct).Materials and MethodsThis was a retrospective cohort analysis of 61 consecutive patients undergoing preoperative, day 1 and day 2 Hb and Hct concentration determination following … Show more

“…A detailed search of literature revealed little information as to the timing of postoperative haematocrit following surgery in Obstetrics and Gynaecology. This has been corroborated by authors in other faculties (4,6). In our institutions, we found out that there was an inconsistent timing of post-operative haematocrit; with most measurements being done either on postoperative (POD) 1 or 2.…”

“…This could be due to ongoing blood loss from the wound site, further dilution by intravenous fluids, timing of equilibration of the blood after acute blood loss and atimes inflammatory response to surgery (4). Various studies have shown that reequilibration of blood after acute blood loss or transfusion usually occurs within 24 hours (6,10,11). This implies that the difference noticed was due to ongoing blood loss.…”

“…Total knee arthroplasty (TKA) is the preferred treatment option for knee osteoarthritis. [6][7][8][9][10][11] As the technique has evolved, many different types of knee prostheses have emerged. However, the choice of prosthesis for TKA remains controversial as surgeons seek to provide patients with a stable implant that offers adequate range of motion (ROM) with minimal risk for postoperative complications or revision surgery.…”
