Optimal Route for Tranexamic Acid in Diabetics and Obese Patients Undergoing Primary Total Knee Arthroplasty - a Data from Randomized Study

Abstract: PURPOSE OF THE STUDYTo determine the optimal strategy for tranexamic acid (TXA) administration in diabetic patients, smokers and obese patients (BMI > 30 kg/m 2 ) undergoing primary total knee arthroplasty (TKA). MATERIAL AND METHODSThe total of 400 consecutive patients indicated for primary TKA were randomised into 4 basic groups with different TXA administration regimens. Group 1 (IV1) had a single intravenous dose (15 mg TXA/kg) applied prior to skin incision. Group 2 (IV2) got two intravenous doses (15 mg … Show more

“…Legend: 1-NO TXA, 2 -IV TXA, 3 -Topical TXA 15) analysed an optimal strategy of TXA administration in diabetic and obese patients undergoing primary TKA. Two IV doses and the combined topical/IV administration of TXA was more effective than topical administration (15). A secondary benefit of TXA is reduction of the heterotopic ossification ratio after primary THA (5).…”

“…Proto se v současné době u rozsáhlých operačních výkonů, zejména korekčních ke kontrole krevních ztrát a k detekci případné perioperační koagulopatie využívá rotační trombelastometrie (14). Ve velkých ortopedických operacích byl pozorován pozitivní přínos optimálního peroperačního podávání tranexamové kyseliny na vznik hematomu, hemarthros a následné sekundární infekční komplikaci zvláště u diabetických pacientů (19). V našem souboru jsme zaznamenali 0,88 % případů hematomů.…”
