Optical Coherence Tomography: An Eye Into the Coronary Artery

Abstract: Optical coherence tomography (OCT) is slowly but surely gaining a foothold in the hands of interventional cardiologists. Intraluminal and transmural contents of the coronary arteries are no longer elusive to the cardiologist's probing eye. Although the graduation of an interventionalist in imaging techniques right from naked eye angiographies to ultrasound-based coronary sonographies to the modern light-based OCT has been slow, with the increasing regularity of complex coronary cases in practice, such a transi… Show more

“…ICI, especially OCT, plays an important role in the identification of causes of stent failure like under expanded stent, reason for underexpansion, stent fracture, geographical miss, and a rare cause as shown in this case 4 . OCT was critical in this case to identify the embolized broken guiding tip as the cause of stent failure by showing the “eclipse sign” as seen with covered stent 5 .…”

“…Although, the broken guiding tip was embedded inside NIH, an attempt was made to remove it using a shown in this case. 4 OCT was critical in this case to identify the embolized broken guiding tip as the cause of stent failure by showing the "eclipse sign" as seen with covered stent. 5 We don't think without ICI, this would have been diagnosed.…”

“…Although optical coherence tomography (OCT) offers greater axial resolution compared to IVUS, penetrating depth is lower. OCT is of limited benefit in assessment of large CAA [ 15 ]. Coronary CTA offers advantages due to more accurate assessment of the aneurysm size and degree of thrombus verses invasive angiography.…”

“…The overall procedural approach for OCT-guided stent implantation and post-implant optimization also follows this algorithm (18). In short, the stent size is chosen based on OCT measurement, and post-dilation performed under OCT guidance to achieve fully stent struts apposition and satisfactory stent area (19).…”
