Optic Disc Tomography and Perimetry in Controls, Glaucoma Suspects, and Early and Established Glaucomas

Abstract: Perimetry reveals age-related defects slightly more than HRT II. HRT II indices correlated better with MS than with MD, probably because none of them was age corrected. Good correlation between anatomical and functional data in C and ESG, and sensitivity results indicated small differences in early development between both methods. Perimetry identifies defects more clearly than structural measures in established glaucoma.

“…Several of our previous studies 31,32 have cast doubt on the hypothesis that morphological defects habitually precede functional defects, which has been defended by numerous authors. In our opinion, they are phenomena which occur in parallel, and defects can sometimes be detected earlier with one or another system, largely depending on the diagnostic procedure chosen.…”

“…Since publication of the work by Quigley et al (19), much has been written about the sequence of events leading to glaucomatous damage and the methods used for early detection. The general opinion, represented by the Glaucoma Continuum (20), is that defects detected by SAP are late manifestations, usually preceded by a phase of preperimetric glaucoma, but for many years it was not easy to defend the idea that there may be 2 parallel phenomena (2).…”

“…Numerous comparative studies have been performed using confocal tomography (Heidelberg Retina Tomograph) (13), scanning laser polarimetry (48), or optical coherence tomography (OCT) (913). Since each ganglion cell captures information from a particular region of the visual field, most researchers have focused their studies on the relationship between the sensitivities of visual field areas with the corresponding anatomical sectors of the nerve fiber layer, at a circular region parallel to the optic nerve head (ONH), or with the corresponding sectors of the neuroretinal rim.…”
