Operationalization of a frailty index among older adults in the InCHIANTI study: predictive ability for all-cause and cardiovascular disease mortality

Abstract: Background The frailty index (FI) is a sensitive instrument to measure the degree of frailty in older adults, and is increasingly used in cohort studies on aging. Aims To operationalize an FI among older adults in the "Invecchiare in Chianti" (InCHIANTI) study, and to validate its predictive capacity for mortality. Methods Longitudinal data were used from 1129 InCHIANTI participants aged ≥ 65 years. A 42-item FI was operationalized following a standard procedure using baseline data (1998/2000). Associations of… Show more

“…Cox proportional hazard models (sex-adjusted) estimated hazard ratios and 95% confidence intervals (HR(95%CI)) of associations between the FI 50y and all-cause mortality between ages 50y-58y. Associations between FI and mortality are commonly examined using a continuous measure [36] or FI categories derived from specific cut-points (e.g. 0-0.1, 0.1-0.2, etc) [2,37,38].…”

“…In the InCHIANTI study, an Italian prospective cohort study, the FI was operationalized using 42 variables that reflected age-related disease diagnosis, physical function, and cognitive health [ 3 ]. The FI at baseline was predictive of all-cause and cardiovascular disease mortality [ 3 ].…”

“…The frailty index (FI) is an often-used instrument to measure frailty, and it is based on the accumulation of deficits [7]. And while it has mostly been used to measure (gradations in) frailty, the FI is a sensitive measure and it encompasses the entire continuum, from fit to frail [12]. In the current study, we will, therefore, use the FI as a measure of frailty as well as vitality.…”
