Estimation of the Burden of Major Cancers in Korea

We estimated the burden of diseases in Korea especially caused by major cancers using DALY (disability adjusted life year) measurement. Firstly, the burden of disease due to premature death was estimated by using YLLs (years life lost due to premature death) measurement developed by the global burden of disease study group. Secondly, for the calculation of the YLD (years lived with disability), the following parameters were estimated in the formula; incidence rate, case fatality rate and disability weight of major cancers. Thirdly, we estimated DALY of major cancers by adding YLLs and YLDs. The burden of major cancers for male per 100,000 population was attributed mainly to liver cancer (528.8 person-year), stomach cancer (451.4 person-year), and lung cancer (374.9 personyear). The burden of major cancers for female per 100,000 population was attributed mainly to liver cancer (140.0 person-year), stomach cancer (259.7 personyear), and lung cancer (125.2 person-year). Each of these cancers was responsible for the loss of over 100 person-year per 100,000 population based on our DALY measurement. We found the DALY method employed was appropriate to quantify the burden of disease. Thereby, it would provide a rational bases to plan a national health policy regarding the burden of disease caused by major cancers in Korea

Environmental epidemiology and health risk and impact assessment have long grappled with problems of uncertainty in data and their relationships. These uncertainties have become more challenging because of the complex, systemic nature of many of the risks. A clear framework defining and quantifying uncertainty is needed. Three dimensions characterise uncertainty: its nature, its location and its level. In terms of its nature, uncertainty can be both intrinsic and extrinsic. The former reflects the effects of complexity, sparseness and nonlinearity; the latter arises through inadequacies in available observational data, measurement methods, sampling regimes and models. Uncertainty occurs in three locations: conceptualising the problem, analysis and communicating the results. Most attention has been devoted to characterising and quantifying the analysis-a wide range of statistical methods has been developed to estimate analytical uncertainties and model their propagation through the analysis. In complex systemic risks, larger uncertainties may be associated with conceptualisation of the problem and communication of the analytical results, both of which depend on the perspective and viewpoint of the observer. These imply using more participatory approaches to investigation, and more qualitative measures of uncertainty, not only to define uncertainty more inclusively and completely, but also to help those involved better understand the nature of the uncertainties and their practical implications.

O trabalho faz uma avaliação da proposição de que o conceito de 'saúde' constitui um ponto-cego para a ciência epidemiológica, postulando que não há base lógica para uma definição negativa da Saúde, tanto no nível individual quanto no coletivo. Também analisa brevemente as tentativas de produzir uma "epidemiologia da saúde" em bases simétricas à epidemiologia dos riscos, bem como as abordagens econométricas que reforçam e complementam o repertório epidemiológico destinado à medida da saúde. Focaliza em mais detalhe a abordagem denominada DALY, considerada como protótipo da nova geração de indicadores de saúde, face à sua atualidade e crescente importância na definição de políticas de financiamento em saúde. Finalmente, constatando o fracasso das propostas metodológicas de avaliação direta dos níveis coletivos de saúde através de indicadores unificados, conclui com uma avaliação das perspectivas atuais da Epidemiologia no sentido da incorporação do objeto complexo da saúde na sua pauta teórica e metodológica.
